Vision in one eye 5/200 (1.5/60) — VA Diagnostic Code 6065
Eye
Diagnostic code
6065
38 CFR § 4.79, under Impairment of Central Visual Acuity, lists Diagnostic Code 6065 as “Vision in one eye 5/200 (1.5/60):”. The other-eye rows run from 100 down to 30. Superscript 1 is on the 100, not on the heading. Appendix B to Part 4, version date 2026-10-01, does not use 6065 for that heading. Under “Anatomical loss of 1 eye:” it prints 6065 three times, including “Other eye 20/50 (6/15).” The catalog slug for this page was built from that index line. In the rating table, anatomical loss of one eye is DC 6063, and “In the other eye 20/50 (6/15)” on that code is 50.

Superscript 1 is on the DC 6065 rating of 100, for 5/200 in the other eye. The DC 6065 heading does not carry that superscript. Footnote 1 of the central-acuity table reads: Review for entitlement to special monthly compensation under 38 CFR 3.350.
The DC 6063 heading does carry superscript 1: “Anatomical loss of one eye:”. Its “In the other eye 20/50 (6/15)” cell is 50. The DC 6065 cell with the same other-eye words is 40, and the first eye on that row is 5/200 (1.5/60), not anatomical loss.
Appendix B says “Vision in 1 eye” and “Other eye,” and it puts a period after (6/15). Section 4.79 says “Vision in one eye” and “In the other eye,” and those rating-table cells do not end with a period. Appendix B uses the numeral “1” in “Anatomical loss of 1 eye:”. The rating table says “one eye.”
Appendix B’s heading “Vision in 1 eye 5/200 (1.5/60):” uses 6071 through 6074, not 6065. Those index numbers are not the DC 6065 rows in the rating table.
38 CFR § 4.75(c) says that, subject to 38 CFR 3.383(a), if visual impairment of only one eye is service-connected, the visual acuity of the other eye will be considered to be 20/40 for purposes of evaluating the service-connected visual impairment. Paragraph (d) says the evaluation for visual impairment of one eye must not exceed 30 percent unless there is anatomical loss of the eye. The DC 6065 cell for 20/40 (6/12) in the other eye is 30. The higher DC 6065 cells name a worse other eye. Paragraph (d) does not name a diagnostic code. Anatomical loss of one eye is the DC 6063 heading. Paragraph (f) says that when evaluating visual impairment, refer to 38 CFR 3.350. This page does not state a payment amount.
38 CFR § 4.76(b)(1) says to evaluate central visual acuity on the basis of corrected distance vision with central fixation, even if a central scotoma is present. Paragraph (b)(4) says that where a reported visual acuity is between two sequentially listed visual acuities, use the visual acuity which permits the higher evaluation. 38 CFR § 4.78(b)(1) says an evaluation for diplopia will be assigned to only one eye, and it names diagnostic codes 6065 through 6066 as the table used after the step adjustment that paragraph describes. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.
This catalog slug is the Appendix B line “Other eye 20/50 (6/15).” The same index also prints 6065 for “Other eye 20/100 (6/30).” and “Other eye 20/70 (6/21).” Those two lines have their own catalog pages, 20/100 and 20/70. This page does not replace them. DC 6062 is no more than light perception in both eyes. DC 6066 is the catalog page built from “Other eye 20/40 (6/12).” under anatomical loss of 1 eye in the index. In the rating table, DC 6066 is “Visual acuity in one eye 10/200 (3/60) or better:”.

What the words mean in your records
Two official uses of the number 6065
The rating table assigns 6065 to vision in one eye of 5/200 (1.5/60). Appendix B assigns 6065 to three “Other eye” lines under anatomical loss of 1 eye. The 20/50 line is 40 percent on the DC 6065 grid and 50 percent on the DC 6063 grid.
1 eye and one eye
The index uses the numeral “1.” The rating table uses the word “one.” The index says “Other eye.” The rating table says “In the other eye.”
The 100 carries the footnote
On DC 6065, superscript 1 sits on the 100 for 5/200 in the other eye. Footnote 1 says to review special monthly compensation under 38 CFR 3.350. The 40 percent cell for 20/50 in the other eye does not carry that superscript.
Evidence that speaks to each level
The DC 6065 pair
What the rater needs to see. Corrected distance vision of 5/200 (1.5/60) in one eye and one of the other-eye acuities the DC 6065 rows list, from 5/200 through 20/40.
Where that usually lives. The eye examination § 4.75(b) and § 4.76 describe, from a licensed optometrist or ophthalmologist.
The index line on this slug
What the rater needs to see. If the record is anatomical loss of one eye with 20/50 (6/15) in the other eye, that pair is a DC 6063 cell at 50, with superscript 1 on the DC 6063 heading. It is not the DC 6065 cell at 40.
Where that usually lives. The same examination, read against which eye was lost and the acuity in the other eye.
VA’s public DBQ list, checked October 5, 2026, names “Eye Conditions” under Ophthalmological. The file is Eye Conditions.
The exam form
The percentages in the first table on this page are DC 6065 cells. The 50 percent cell in the third table is a DC 6063 cell. This page does not say the form chooses between those codes.
Related codes and how they interact
The 20/40 other-eye line is a different index number.
Under anatomical loss of 1 eye, Appendix B uses 6066, not 6065, for “Other eye 20/40 (6/12).” In the rating table, the DC 6065 row for 20/40 in the other eye is 30, and the first eye is 5/200.
Secondary conditions and presumptions
The 2026-10-01 text of 38 CFR § 3.309 does not contain “6065,” “5/200,” or “20/50.” It does not assign this code. The DC 6065 footnote on the 100 points to § 3.350, not to § 3.309.
Short answers
What is the VA rating for DC 6065?
In the 2026-10-01 text of 38 CFR § 4.79, Diagnostic Code 6065 is vision in one eye 5/200 (1.5/60), from 100 to 30 depending on the other eye. Education only. VA decides on the evidence.
Where is other eye 20/50 under anatomical loss rated?
DC 6063, “Anatomical loss of one eye:”, “In the other eye 20/50 (6/15),” is 50. Superscript 1 is on that DC 6063 heading. Appendix B still numbers that index line 6065.
FAQ
What does diagnostic code 6065 rate in the current table?
Vision in one eye 5/200 (1.5/60). The other eye is 100 at 5/200, 90 at 10/200, 80 at 15/200, 70 at 20/200, 60 at 20/100, 50 at 20/70, 40 at 20/50, and 30 at 20/40. Superscript 1 is on the 100. Education only.
Why does this page’s address say other eye 20/50?
Appendix B still prints “6065 Other eye 20/50 (6/15).” under anatomical loss of 1 eye. The catalog slug follows that index line. In § 4.79, that other-eye pair under anatomical loss of one eye is a DC 6063 row at 50. The DC 6065 row that says 20/50 in the other eye is 40, and the first eye is 5/200.
Are the other two Appendix B 6065 lines this page?
They are the same index number, under the same “Anatomical loss of 1 eye” heading, for 20/100 and 20/70. Each has its own catalog slug. This page is the 20/50 slug. The rating-table meaning of DC 6065 is the 5/200 grid, which includes all three of those other-eye words at 60, 50, and 40.
