Burn scar(s) of the head, face, or neck; scar(s) of the head, face, or neck due to other causes; or other disfigurement of the head, face, or neck — VA Diagnostic Code 7800

Skin

Diagnostic code

7800

38 CFR § 4.118 lists Diagnostic Code 7800 as “Burn scar(s) of the head, face, or neck; scar(s) of the head, face, or neck due to other causes; or other disfigurement of the head, face, or neck:”. The percentage lines are 80, 50, 30, and 10. None of those four cells ends with a period. Note (1) lists eight characteristics of disfigurement. Appendix B to Part 4 uses the same long title and ends it with a period.

Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.118 source note prints 67 FR 49596, July 31, 2002; 67 FR 58448, 58449, Sept. 16, 2002; 73 FR 54710, Oct. 23, 2008; 77 FR 2910, Jan. 20, 2012; 83 FR 32597, July 13, 2018; 83 FR 38663, Aug. 7, 2018.

The rating criteria at each level

7800 Burn scar(s) of the head, face, or neck; scar(s) of the head, face, or neck due to other causes; or other disfigurement of the head, face, or neck

Rating %: 80

Criteria (verbatim). With visible or palpable tissue loss and either gross distortion or asymmetry of three or more features or paired sets of features (nose, chin, forehead, eyes (including eyelids), ears (auricles), cheeks, lips), or; with six or more characteristics of disfigurement

Rating %: 50

Criteria (verbatim). With visible or palpable tissue loss and either gross distortion or asymmetry of two features or paired sets of features (nose, chin, forehead, eyes (including eyelids), ears (auricles), cheeks, lips), or; with four or five characteristics of disfigurement

Rating %: 30

Criteria (verbatim). With visible or palpable tissue loss and either gross distortion or asymmetry of one feature or paired set of features (nose, chin, forehead, eyes (including eyelids), ears (auricles), cheeks, lips), or; with two or three characteristics of disfigurement

Rating %: 10

Criteria (verbatim). With one characteristic of disfigurement

Note (1): The 8 characteristics of disfigurement, for purposes of evaluation under § 4.118, are:

Scar 5 or more inches (13 or more cm.) in length.

Scar at least one-quarter inch (0.6 cm.) wide at widest part.

Surface contour of scar elevated or depressed on palpation.

Scar adherent to underlying tissue.

Skin hypo-or hyper-pigmented in an area exceeding six square inches (39 sq. cm.).

Skin texture abnormal (irregular, atrophic, shiny, scaly, etc.) in an area exceeding six square inches (39 sq. cm.).

Underlying soft tissue missing in an area exceeding six square inches (39 sq. cm.).

Skin indurated and inflexible in an area exceeding six square inches (39 sq. cm.).

The pigment sentence prints “hypo-or” with a hyphen and no space. The square-inch lines use “sq. cm.” with periods.

Note (2): Rate tissue loss of the auricle under DC 6207 (loss of auricle) and anatomical loss of the eye under DC 6061 (anatomical loss of both eyes) or DC 6063 (anatomical loss of one eye), as appropriate.

Note (3): Take into consideration unretouched color photographs when evaluating under these criteria.

Note (4): Separately evaluate disabling effects other than disfigurement that are associated with individual scar(s) of the head, face, or neck, such as pain, instability, and residuals of associated muscle or nerve injury, under the appropriate diagnostic code(s) and apply § 4.25 to combine the evaluation(s) with the evaluation assigned under this diagnostic code.

Note (5): The characteristic(s) of disfigurement may be caused by one scar or by multiple scars; the characteristic(s) required to assign a particular evaluation need not be caused by a single scar in order to assign that evaluation.

The 80 and 50 percent lines say “features or paired sets of features.” The 30 percent line says “one feature or paired set of features.”

38 CFR § 4.118(b) says two or more skin conditions may be combined in accordance with § 4.25 only if separate areas of skin are involved. If two or more skin conditions involve the same area of skin, then only the highest evaluation shall be used. Note (4) of DC 7800 is a different sentence: it names disabling effects other than disfigurement and tells the reader to apply § 4.25 to combine those evaluations with the DC 7800 evaluation. Paragraph (a) defines systemic therapy and topical therapy. DC 7800 does not use those words. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. DC 7800 does not print a 0.

DC 7801 is burn scars or scars due to other causes, not of the head, face, or neck, that are associated with underlying soft tissue damage. DC 7804 is scar(s), unstable or painful. Its Note (3) says scars evaluated under diagnostic codes 7800, 7801, 7802, or 7805 may also receive an evaluation under that code, when applicable. DC 7805 says to evaluate any disabling effect(s) not considered in a rating provided under diagnostic codes 7800-04 under an appropriate diagnostic code. Note (2) of DC 7800 names DC 6207, DC 6061, and DC 6063 for tissue loss of the auricle and anatomical loss of the eye.

The 2026-10-01 text of 38 CFR § 3.309 does not contain “scar.” It does not assign DC 7800.

What the words mean in your records

Eight characteristics

Note (1) lists eight. The 80 percent line uses six or more. The 50 percent line uses four or five. The 30 percent line uses two or three. The 10 percent line is one characteristic of disfigurement.

One scar or many

Note (5) says the characteristics required for a particular evaluation need not be caused by a single scar.

Photographs

Note (3) says to take into consideration unretouched color photographs when evaluating under these criteria.

Evidence that speaks to each level

80, 50, or 30

What the rater needs to see. Visible or palpable tissue loss and the gross distortion or asymmetry of the number of features or paired sets the line names, or the characteristic count that line names.

Where that usually lives. The examination of the nose, chin, forehead, eyes including eyelids, ears (auricles), cheeks, and lips, and the eight characteristics in Note (1).

10

What the rater needs to see. One characteristic of disfigurement.

Where that usually lives. The same characteristic list.

Note (3)

What the rater needs to see. Unretouched color photographs, when they are part of the evaluation under these criteria.

Where that usually lives. The photograph set the note names.

Note (4)

What the rater needs to see. Disabling effects other than disfigurement, such as pain, instability, and residuals of associated muscle or nerve injury, evaluated under the appropriate diagnostic code and combined under § 4.25.

Where that usually lives. The findings for those effects, separate from the disfigurement count.

VA’s public DBQ list, checked October 5, 2026, names “Scars” under Dermatological. The file is Scars.

The exam form

DC 7800 prints 80, 50, 30, and 10, and Note (1) lists eight characteristics. This page does not say the form replaces those lines.

Related codes and how they interact

Scars that are not of the head, face, or neck are DC 7801.

DC 7801 is associated with underlying soft tissue damage and is not of the head, face, or neck. DC 7804 is unstable or painful scars. Its Note (3) names DC 7800. DC 7805 covers other disabling effects of scars evaluated under DC 7800, 7801, 7802, or 7804.

Secondary conditions and presumptions

Section 3.309 does not contain “scar,” and it does not assign DC 7800. This page does not add DC 7800 to a presumptive row.

A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

Short answers

How does VA rate scars of the head, face, or neck?

Under 38 CFR § 4.118, Diagnostic Code 7800 is 80, 50, 30, or 10. The title also covers scars of the head, face, or neck due to other causes, and other disfigurement of the head, face, or neck. Note (1) lists eight characteristics of disfigurement. Education only. VA decides on the evidence.

What does Note (2) do with the ear and the eye?

It says to rate tissue loss of the auricle under DC 6207 and anatomical loss of the eye under DC 6061 or DC 6063, as appropriate. Those are not the 80, 50, 30, and 10 lines.

FAQ

What are the ratings for head, face, or neck scars under DC 7800?

80 with tissue loss and distortion or asymmetry of three or more features or paired sets, or with six or more characteristics of disfigurement. 50 with two features or paired sets, or four or five characteristics. 30 with one feature or paired set, or two or three characteristics. 10 with one characteristic of disfigurement. The code does not print a 0. Education only.

How many characteristics of disfigurement does Note (1) list?

Eight. They are length, width, surface contour, adherence, pigment in an area exceeding six square inches, abnormal texture in that area, missing underlying soft tissue in that area, and indurated and inflexible skin in that area. The pigment line prints “hypo-or” with a hyphen.

Does one scar have to cause every characteristic?

Note (5) says the characteristics may be caused by one scar or by multiple scars, and the characteristics required to assign a particular evaluation need not be caused by a single scar.
