38 CFR Part 4 — 38 CFR § 4.118

Burn scars with underlying soft tissue damage, not of the head, face, or neck — VA Diagnostic Code 7801

Skin

Diagnostic code

7801

38 CFR § 4.118 lists Diagnostic Code 7801 as “Burn scar(s) or scar(s) due to other causes, not of the head, face, or neck, that are associated with underlying soft tissue damage:”. The title ends with a colon. The rating cell on that title is empty. The area lines are 40, 30, 20, and 10. Two notes follow. Appendix B to Part 4 drops the comma after “neck” and ends with a period.

Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.118 source note prints 67 FR 49596, July 31, 2002; 67 FR 58448, 58449, Sept. 16, 2002; 73 FR 54710, Oct. 23, 2008; 77 FR 2910, Jan. 20, 2012; 83 FR 32597, July 13, 2018; 83 FR 38663, Aug. 7, 2018.

The rating criteria at each level

7801 Burn scar(s) or scar(s) due to other causes, not of the head, face, or neck, that are associated with underlying soft tissue damage

Rating %: 40

Criteria (verbatim). Area or areas of 144 square inches (929 sq. cm.) or greater

Rating %: 30

Criteria (verbatim). Area or areas of at least 72 square inches (465 sq. cm.) but less than 144 square inches (929 sq. cm.)

Rating %: 20

Criteria (verbatim). Area or areas of at least 12 square inches (77 sq. cm.) but less than 72 square inches (465 sq. cm.)

Rating %: 10

Criteria (verbatim). Area or areas of at least 6 square inches (39 sq. cm.) but less than 12 square inches (77 sq. cm.)

Note (1): For the purposes of DCs 7801 and 7802, the six (6) zones of the body are defined as each extremity, anterior trunk, and posterior trunk. The midaxillary line divides the anterior trunk from the posterior trunk

Note (2): A separate evaluation may be assigned for each affected zone of the body under this diagnostic code if there are multiple scars, or a single scar, affecting multiple zones of the body. Combine the separate evaluations under § 4.25. Alternatively, if a higher evaluation would result from adding the areas affected from multiple zones of the body, a single evaluation may also be assigned under this diagnostic code

Neither note ends with a period. The area lines do not end with a period. The abbreviation is “sq. cm.” with periods after “sq” and “cm”.

Note (2) gives two methods. A separate evaluation may be assigned for each affected zone if there are multiple scars, or a single scar, affecting multiple zones, and those evaluations are combined under 38 CFR § 4.25. Alternatively, if a higher evaluation would result from adding the areas affected from multiple zones, a single evaluation may also be assigned under this diagnostic code. The six zones in Note (1) are each extremity, the anterior trunk, and the posterior trunk. The midaxillary line divides the anterior trunk from the posterior trunk. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.

DC 7802 uses the same “not of the head, face, or neck” words, then says the scars “are not associated with underlying soft tissue damage.” Its only percentage is 10, for an area or areas of 144 square inches (929 sq. cm.) or greater. It reprints Note (1) and Note (2). DC 7800 is the head, face, or neck code. DC 7804 is “Scar(s), unstable or painful.” Note (3) under DC 7804 says scars evaluated under diagnostic codes 7800, 7801, 7802, or 7805 may also receive an evaluation under that diagnostic code, when applicable. That note does not end with a period. DC 7805 says to evaluate any disabling effect(s) not considered in a rating provided under diagnostic codes 7800-04 under an appropriate diagnostic code.

Section 4.118(b) says two or more skin conditions may be combined in accordance with § 4.25 only if separate areas of skin are involved. If two or more skin conditions involve the same area of skin, then only the highest evaluation shall be used. 38 CFR § 4.14 says the evaluation of the same disability under various diagnoses is to be avoided, and that the evaluation of the same manifestation under different diagnoses is to be avoided. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. DC 7801’s lowest printed area line is 10. It does not print a 0 percent line.

What the words mean in your records

Associated with underlying soft tissue damage

Those words are in the DC 7801 title. DC 7802 is the code for scars that are not associated with underlying soft tissue damage. The comma after “neck” is in the schedule title. Appendix B omits that comma.

The area steps

40 is 144 square inches (929 sq. cm.) or greater. 30 starts at 72 square inches (465 sq. cm.) and stays under 144. 20 starts at 12 square inches (77 sq. cm.) and stays under 72. 10 starts at 6 square inches (39 sq. cm.) and stays under 12.

Zones, or one added area

Note (2) allows a separate evaluation for each affected zone, combined under § 4.25, or one evaluation if adding the areas from multiple zones would result in a higher evaluation.

Evidence that speaks to each level

40, 30, 20, or 10

What the rater needs to see. The area or areas in square inches or square centimeters, and that the scar is associated with underlying soft tissue damage and is not of the head, face, or neck.

Where that usually lives. The scar examination that measures the area and names the zone.

More than one zone

What the rater needs to see. Whether each affected zone is evaluated separately and combined under § 4.25, or the areas are added into one evaluation because that method results in a higher evaluation. Note (2) states both methods.

Where that usually lives. The same examination, read with Note (1)’s six zones.

VA’s public DBQ list, checked October 5, 2026, names “Scars” under Dermatological. The file is Scars.

The exam form

DC 7801 prints the four area lines and two notes. This page does not say the form replaces those lines.

Related codes and how they interact

The scar that is not associated with soft tissue damage is DC 7802.

DC 7802 prints 10 percent only, and only at 144 square inches (929 sq. cm.) or greater. DC 7801 prints 40, 30, 20, and 10 for scars that are associated with underlying soft tissue damage.

Secondary conditions and presumptions

The 2026-10-01 text of 38 CFR § 3.309 does not contain “scar.” This page does not add DC 7801 to a presumptive row.

A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

Short answers

What is the VA rating for scars with underlying soft tissue damage?

Under 38 CFR § 4.118, Diagnostic Code 7801 is 40, 30, 20, or 10 percent by area, for scars not of the head, face, or neck that are associated with underlying soft tissue damage. Note (2) addresses more than one zone of the body. Education only. VA decides on the evidence.

How does Appendix B differ from the schedule title?

The schedule title has a comma after “neck” and ends with a colon. Appendix B says “not of the head, face, or neck that are associated with underlying soft tissue damage.” It drops that comma and ends with a period.

FAQ

What percentages does DC 7801 print?

40, 30, 20, and 10, by the area of burn scars or other scars that are not of the head, face, or neck and are associated with underlying soft tissue damage. The smallest printed line starts at 6 square inches (39 sq. cm.). Education only.

Do the notes end with a period?

No. Note (1) ends with “posterior trunk.” Note (2) ends with “under this diagnostic code.” Neither prints a final period.

Can an unstable or painful scar use another code?

Note (3) under DC 7804 says scars evaluated under diagnostic codes 7800, 7801, 7802, or 7805 may also receive an evaluation under DC 7804, when applicable. That sentence is printed under DC 7804.
