Myelodysplastic syndromes — VA Diagnostic Code 7725
Hematologic / lymphatic
Diagnostic code
7725
38 CFR § 4.117 lists Diagnostic Code 7725 as “Myelodysplastic syndromes.” The percentage lines are 100, 60, and 30. Note (2) assigns 100 percent from hospital admission for stem-cell transplant, or during chemotherapy, and requires an examination six months later. Section 3.320b(b)(4) says “Myelodysplastic Syndromes (MDS).” The schedule title does not print the abbreviation or the capital S. Section 3.309(f)(7) is a different sentence: “Aplastic anemia and other myelodysplastic syndromes.”

The rating criteria at each level
Myelodysplastic syndromes — DC 7725
Rating %: 100
Criteria (verbatim). Requiring peripheral blood or bone marrow stem cell transplant; or requiring chemotherapy
Rating %: 60
Criteria (verbatim). Requiring 4 or more blood or platelet transfusions per 12-month period; or infections requiring hospitalization 3 or more times per 12-month period
Rating %: 30
Criteria (verbatim). Requiring at least 1 but no more than 3 blood or platelet transfusions per 12-month period; infections requiring hospitalization at least 1 but no more than 2 times per 12-month period; or requiring biologic therapy on an ongoing basis or erythropoiesis stimulating agent (ESA) for 12 weeks or less per 12-month period
| Rating % | Criteria (verbatim) |
|---|---|
| 100 | Requiring peripheral blood or bone marrow stem cell transplant; or requiring chemotherapy |
| 60 | Requiring 4 or more blood or platelet transfusions per 12-month period; or infections requiring hospitalization 3 or more times per 12-month period |
| 30 | Requiring at least 1 but no more than 3 blood or platelet transfusions per 12-month period; infections requiring hospitalization at least 1 but no more than 2 times per 12-month period; or requiring biologic therapy on an ongoing basis or erythropoiesis stimulating agent (ESA) for 12 weeks or less per 12-month period |
Note (1): If the condition progresses to leukemia, evaluate as leukemia under diagnostic code 7703
Note (2): A 100 percent evaluation shall be assigned as of the date of hospital admission for peripheral blood or bone marrow stem cell transplant, or during the period of treatment with chemotherapy, and shall continue with a mandatory VA examination six months following hospital discharge or, in the case of chemotherapy treatment, six months after completion of treatment. Any reduction in evaluation based upon that or any subsequent examination shall be subject to the provisions of § 3.105(e) of this chapter. If there has been no recurrence, residuals will be rated under the appropriate diagnostic codes
DC 7725 does not print a 0 percent line. 38 CFR § 4.31 says: “In every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met.” 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.
The 60 percent line joins its two clauses with “or.” The 30 percent line puts a semicolon, not the word “or,” between the transfusion clause and the infection clause, and then uses “or” before biologic therapy or an erythropoiesis stimulating agent. This page quotes that punctuation.
Note (1) has no period at the end in the 2026-10-01 text. Note (2) ends on “diagnostic codes” without a period.

What the words mean in your records
Stem-cell transplant or chemotherapy, and the six-month examination
The 100 percent line is “Requiring peripheral blood or bone marrow stem cell transplant; or requiring chemotherapy.” Note (2) assigns that 100 percent as of the date of hospital admission for the transplant, or during the period of treatment with chemotherapy. It continues with a mandatory VA examination six months following hospital discharge or, for chemotherapy, six months after completion of treatment. A reduction based on that examination, or a later one, follows 38 CFR § 3.105(e). If there has been no recurrence, residuals are rated under the appropriate diagnostic codes.
Transfusions and infections
The 60 percent line is 4 or more blood or platelet transfusions per 12-month period, or infections requiring hospitalization 3 or more times per 12-month period. The 30 percent transfusion clause is at least 1 but no more than 3 per 12-month period. The infection clause on that line is hospitalization at least 1 but no more than 2 times per 12-month period. The third clause is biologic therapy on an ongoing basis, or an erythropoiesis stimulating agent (ESA) for 12 weeks or less per 12-month period.
Progression to leukemia
Note (1) says: “If the condition progresses to leukemia, evaluate as leukemia under diagnostic code 7703.” DC 7703 is a different code. This page does not restate its lines.
Evidence that speaks to each level
100
What the rater needs to see. A peripheral blood or bone marrow stem cell transplant, or chemotherapy. Note (2) uses the hospital admission date for the transplant and the treatment period for chemotherapy, then the six-month examination.
Where that usually lives. The transplant admission record or the chemotherapy record, and the later examination.
60 or 30
What the rater needs to see. The number of blood or platelet transfusions in a 12-month period, the number of infections requiring hospitalization in that period, or, at 30 percent, ongoing biologic therapy or an ESA for 12 weeks or less in that period.
Where that usually lives. Transfusion records, hospitalization records, and the medication list.
VA’s public DBQ list, checked October 5, 2026, names “Hematologic and Lymphatic Conditions, Including Leukemia” under Hematologic & Lymphatic. The file is Hematologic and Lymphatic Conditions, Including Leukemia.
The exam form
DC 7725’s lines are the 100, 60, and 30 percent treatment findings and the two notes. This page does not say the form replaces that text.
Related codes and how they interact
Aplastic anemia is DC 7716, not this code
DC 7716 is “Aplastic anemia.” DC 7725 is “Myelodysplastic syndromes.” Section 3.309(f)(7) puts both ideas in one sentence. This page does not treat that sentence as a direction to use only DC 7725.
Secondary conditions and presumptions
38 CFR § 3.320b is titled “Presumptive service connection for leukemias, multiple myelomas, myelodysplastic syndromes, and myelofibrosis.” Paragraph (b)(4) is “Myelodysplastic Syndromes (MDS).” Paragraph (a) uses the covered-veteran definition in § 3.320a(c). This page does not restate that definition. Paragraph (c) is the exception paragraph. The source note is 90 FR 1901, Jan. 10, 2025. The authority line prints 38 U.S.C. 501, 1119, 1171, 1172, 1173, and 1174.
38 U.S.C. § 1120(b)(2), read on the Legal Information Institute page on 2026-10-05, lists types of cancer and does not use the words “myelodysplastic” or “MDS.” 38 CFR § 3.320 does not name myelodysplastic syndromes. Section 3.309(e) does not name them. Section 3.309(a) does not name them.
38 CFR § 3.309(f)(7) says “Aplastic anemia and other myelodysplastic syndromes.” That sentence names aplastic anemia and then “other myelodysplastic syndromes.” The schedule title at DC 7716 is “Aplastic anemia.” The schedule title at DC 7725 is “Myelodysplastic syndromes.” This page does not assign the Camp Lejeune sentence to DC 7725 alone.
A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
What is the VA rating for DC 7725?
Under 38 CFR § 4.117, myelodysplastic syndromes are rated 100, 60, or 30 percent. The code does not print a 0 percent line. Note (2) times the 100 percent from hospital admission for stem-cell transplant, or during chemotherapy, and requires an examination six months later. Education only. VA decides on the evidence.
Is MDS on the § 1120 cancer list?
The enumerated list in 38 U.S.C. § 1120(b)(2), read on October 5, 2026, does not use “myelodysplastic” or “MDS.” The regulatory listing read for this page is 38 CFR § 3.320b(b)(4).
FAQ
How does VA rate myelodysplastic syndromes under DC 7725?
A requirement for peripheral blood or bone marrow stem cell transplant, or for chemotherapy, is 100 percent. Four or more blood or platelet transfusions in 12 months, or infections requiring hospitalization 3 or more times in 12 months, is 60 percent. The 30 percent line is at least 1 but no more than 3 transfusions, infections requiring hospitalization at least 1 but no more than 2 times, or ongoing biologic therapy or an ESA for 12 weeks or less, in a 12-month period. Education only.
Where does § 3.320b name MDS?
Paragraph (b)(4) says “Myelodysplastic Syndromes (MDS).” The schedule title is “Myelodysplastic syndromes,” without the abbreviation. The enumerated list in 38 U.S.C. § 1120(b)(2) does not use those words. Section 3.320 does not name them.
Does the Camp Lejeune list use only DC 7725?
Section 3.309(f)(7) says “Aplastic anemia and other myelodysplastic syndromes.” Aplastic anemia is the schedule title at DC 7716. Myelodysplastic syndromes is the schedule title at DC 7725. That Camp Lejeune sentence is not assigned to one code on this page.
