SMC-L: Loss of Both Feet, Both Hands, or Combination

38 USC § 1114(l) • Special Monthly Compensation

One of the highest compensation levels for catastrophic service-connected disabilities

Monthly Addition

~$4,194

⚠️

Critical Information

SMC-L is in addition to your base disability rating compensation. This is not a rating percentage - it's a separate monthly payment added to your total.

Typical Total Compensation:

100% Base Rating ($3,938.58) + SMC-L ($4,194) = $7,931/month

+ Additional amounts for dependents (spouse, children, parents)

Who Qualifies for SMC-L?

You qualify for SMC-L if you have any one of the following service-connected conditions:

🖐️ Anatomical Loss or Loss of Use of Both Hands

Both hands amputated or completely non-functional (at or above the wrist)

See full guide for DC 5106 →

🦶 Anatomical Loss or Loss of Use of Both Feet

Both feet amputated or completely non-functional (at or above the ankle)

🖐️🦶 One Hand AND One Foot

Anatomical loss or loss of use of one hand combined with one foot

👁️ Blindness in Both Eyes

Visual acuity of 5/200 or less in both eyes, with or without corrective lenses

🛏️ Permanently Bedridden

Permanently bedridden or so nearly bedridden as to require constant care

🏠 Regular Aid and Attendance

In need of regular aid and attendance (assistance with daily living activities)

Higher SMC Levels (Beyond L)

If you have additional severe disabilities or need aid and attendance, you may qualify for even higher SMC payments:

SMC-M

~$5,497/mo

Loss of both hands (anatomical or loss of use) with need for aid and attendance

SMC-N

~$4,977/mo

Loss of both feet (anatomical or loss of use) with need for aid and attendance

SMC-O

~$5,497/mo

Loss of one hand and one foot (anatomical or loss of use) with need for aid and attendance

SMC-P, R, S, T, etc.

Varies

Additional levels for multiple severe disabilities, complete immobility, or additional aid & attendance requirements

Important: SMC levels can stack and combine in complex ways. Some veterans with multiple catastrophic injuries receive $8,000-$10,000+/month. Professional representation by a VA attorney or experienced VSO is essential to ensure proper SMC level determination.

How to File for SMC-L

1

File for Your Base Disability Rating

Use VA Form 21-526EZ to file for the underlying condition (e.g., bilateral hand amputation under DC 5106 = automatic 100%).

2

Explicitly Request SMC Evaluation

Use VA Form 21-0847 (Request for Increased Compensation Based on SMC). State clearly which SMC level you believe you qualify for and why.

3

Submit Comprehensive Evidence

Include all medical records documenting the loss, functional capacity evaluations, photos/videos, and aid & attendance evidence if applicable.

4

Work with a Professional

SMC-L alone is worth $50,000+/year. Errors in SMC level determination can cost tens of thousands. Use a VA-accredited attorney or experienced VSO who specializes in catastrophic injury claims.
