38 CFR Part 4 — 38 CFR § 4.124a
Neuritis, obturator nerve — VA Diagnostic Code 8628
Peripheral nerves
Diagnostic code
8628
38 CFR § 4.124a lists “Obturator nerve” and then Diagnostic Code 8628 as “Neuritis.” The row prints no percentage. Section 4.123 rates neuritis on the scale for the nerve involved. The paralysis scale for this nerve is DC 8528. That scale has one Rating column. It prints “Severe to complete” at 10 and “Mild or moderate” at 0. It does not print a separate line that says only “Severe” or only “Moderate.” Appendix B to Part 4 indexes “Neuritis, obturator nerve.”
Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.124a source note prints 29 FR 6718, May 22, 1964, as amended at 40 FR 42540, Sept. 15, 1975; 41 FR 11302, Mar. 18, 1976; 43 FR 45362, Oct. 2, 1978; 54 FR 4282, Jan. 30, 1989; 54 FR 49755, Dec. 1, 1989; 55 FR 154, Jan. 3, 1990; 56 FR 51653, Oct. 15, 1991; 57 FR 24364, June 9, 1992; 70 FR 75399, Dec. 20, 2005; 73 FR 54705, Sept. 23, 2008; 73 FR 69554, Nov. 19, 2008; 76 FR 78824, Dec. 20, 2011; 79 FR 2100, Jan. 13, 2014. The § 4.123 and § 4.124 XML for this date print the neuritis and neuralgia paragraphs and do not print a Federal Register source note.

DC 8628 prints “Neuritis.” with the period at the end of the text. DC 8728 prints “Neuralgia.” and then a line break. Neither row prints a percentage.
The 10 percent cell says “Severe to complete.” The 0 percent cell says “Mild or moderate.” The word between mild and moderate is “or.”
The Diseases of the Peripheral Nerves block says: The term “incomplete paralysis,” with this and other peripheral nerve injuries, indicates a degree of lost or impaired function substantially less than the type picture for complete paralysis given with each nerve, whether due to varied level of the nerve lesion or to partial regeneration. When the involvement is wholly sensory, the rating should be for the mild, or at most, the moderate degree. The ratings for the peripheral nerves are for unilateral involvement; when bilateral, combine with application of the bilateral factor.
38 CFR § 4.123 says: “Neuritis, cranial or peripheral, characterized by loss of reflexes, muscle atrophy, sensory disturbances, and constant pain, at times excruciating, is to be rated on the scale provided for injury of the nerve involved, with a maximum equal to severe, incomplete, paralysis. See nerve involved for diagnostic code number and rating. The maximum rating which may be assigned for neuritis not characterized by organic changes referred to in this section will be that for moderate, or with sciatic nerve involvement, for moderately severe, incomplete paralysis.” The higher maximum is severe, incomplete, paralysis, with commas around “incomplete.” The lower maximum, when those organic changes are not present and the nerve is not the sciatic nerve, is moderate. DC 8528 does not print a line that says only “Severe” or only “Moderate.” The line that contains the word severe also says complete, and that line is 10. The line that contains moderate also says mild, and that line is 0. DC 8528 is the paralysis row. DC 8620 is sciatic neuritis. The moderately severe clause does not name the obturator nerve. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.
38 CFR § 4.124 caps neuralgia at moderate incomplete paralysis, without commas between those three words. On this scale the word moderate is on the 0 percent line, together with mild. DC 8728 prints “Neuralgia.” and no percentage. The tic douloureux sentence in § 4.124 is not the obturator nerve. The 0 percent line is printed. 38 CFR § 4.14 says the evaluation of the same disability under various diagnoses is to be avoided, and that the evaluation of the same manifestation under different diagnoses is to be avoided.
DC 8527 is the internal saphenous nerve. Its lower line says “Mild to moderate,” with the word “to.” DC 8528’s lower line says “Mild or moderate,” with the word “or.” Both lower lines are 0. Both upper lines are “Severe to complete,” at 10. Appendix B indexes DC 8528 as “Obturator nerve, paralysis.” It indexes DC 8628 as “Neuritis, obturator nerve.”
The 2026-10-01 text of 38 CFR § 3.309 does not contain “obturator.” Section 3.309(a) lists “Other organic diseases of the nervous system.” That family name is not assigned to DC 8628. A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

What the words mean in your records
Severe to complete is one line
The 10 percent cell is the phrase “Severe to complete.” The schedule does not split that phrase into a severe percentage and a complete percentage.
Mild or moderate is the 0 line
The 0 percent cell is “Mild or moderate.” Section 4.123’s non-sciatic maximum uses the word moderate. Section 4.124’s neuralgia maximum also uses the word moderate. On this scale that word is on the 0 line.
Or, not to
DC 8528 says “Mild or moderate.” The internal saphenous nerve says “Mild to moderate.” The conjunction is different. Both lines are 0.
Evidence that speaks to each level
The DC 8628 row
What the rater needs to see. Neuritis of the obturator nerve, and whether the organic changes § 4.123 names are present.
Where that usually lives. The neurological examination that identifies the obturator nerve.
The two printed lines
What the rater needs to see. Severe to complete, at 10, or mild or moderate, at 0. The scale does not print a separate incomplete-severe percentage.
Where that usually lives. The same examination. Section 4.123 and § 4.124 name severe and moderate. They do not add a third percentage to this table.
VA’s public DBQ list, checked October 5, 2026, names “Peripheral Nerves” under Neurological. The file is Peripheral Nerves.
The exam form
DC 8628 prints no percentage. Section 4.123 and DC 8528 are the scale. This page does not say the form replaces that text.
Related codes and how they interact
Paralysis of this nerve is DC 8528.
That row prints 10 and 0. DC 8728 is neuralgia. The internal saphenous nerve uses “to” on the lower line. The obturator nerve uses “or.”
Secondary conditions and presumptions
The 2026-10-01 text of 38 CFR § 3.309 does not contain “obturator.” DC 8628 is a rating-schedule line. A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
What is the VA rating for neuritis of the obturator nerve?
Under 38 CFR § 4.124a, Diagnostic Code 8628 prints “Neuritis.” and no percentage. Diagnostic Code 8528 prints “Severe to complete” at 10 percent and “Mild or moderate” at 0 percent. Section 4.123 rates neuritis on that scale, with the maximum it states. Education only. VA decides on the evidence.
Which word does the 0 percent line use, or or to?
The obturator line says “Mild or moderate.” The internal saphenous line says “Mild to moderate.” Both are 0 percent.
FAQ
How does VA rate obturator neuritis under DC 8628?
The DC 8628 row prints no percentage. The paralysis scale at DC 8528 prints “Severe to complete” at 10 and “Mild or moderate” at 0. Section 4.123 caps neuritis at severe, incomplete, paralysis when the organic changes it names are present, and otherwise at moderate. This scale does not print those words as separate percentage lines. Education only.
Does the table print a moderate rating by itself?
No. The 0 percent cell says “Mild or moderate.” The 10 percent cell says “Severe to complete.” There is no row that says only “Moderate” or only “Severe.”
Is the obturator nerve the sciatic nerve?
No. The moderately severe clause in § 4.123 names sciatic nerve involvement. DC 8620 is sciatic neuritis. DC 8628 is the obturator nerve.
