Middle radicular group, paralysis — VA Diagnostic Code 8511
Peripheral nerves
Diagnostic code
8511
38 CFR § 4.124a lists “Middle radicular group,” then Diagnostic Code 8511, “Paralysis of:”. The heading has no parenthetical. The columns are Major and Minor. Complete is 70 and 60. The complete cell names adduction, abduction and rotation of the arm, flexion of the elbow, and extension of the wrist. Appendix B to Part 4 indexes “Middle radicular group, paralysis.” The catalog slug is dc-8511-middle-radicular-group-paralysis.

8611 Neuritis. 8711 Neuralgia. Those two rows sit under DC 8511 and do not print a percentage. Each ends with a period.
The complete cell does not end with a period. It says “abduction and rotation,” with “and” rather than a comma between those two words.
The heading is “Middle radicular group.” It does not add “fifth and sixth cervicals.” That parenthetical is the upper radicular heading, DC 8510.
The Diseases of the Peripheral Nerves block says: The term “incomplete paralysis,” with this and other peripheral nerve injuries, indicates a degree of lost or impaired function substantially less than the type picture for complete paralysis given with each nerve, whether due to varied level of the nerve lesion or to partial regeneration. When the involvement is wholly sensory, the rating should be for the mild, or at most, the moderate degree. The ratings for the peripheral nerves are for unilateral involvement; when bilateral, combine with application of the bilateral factor. That sentence uses “varied level.” It says wholly sensory involvement should be mild, or at most moderate. 38 CFR § 4.26 is the bilateral factor the peripheral-nerve sentence names. 38 CFR § 4.69 says handedness for the purpose of a dominant rating will be determined by the evidence of record, or by testing on VA examination. Only one hand shall be considered dominant. Section 4.69 does not use the words “Major” and “Minor.” Those are the column headings. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just. 38 CFR § 4.14 says the evaluation of the same disability under various diagnoses is to be avoided, and that the evaluation of the same manifestation under different diagnoses is to be avoided.
38 CFR § 4.123 says: “Neuritis, cranial or peripheral, characterized by loss of reflexes, muscle atrophy, sensory disturbances, and constant pain, at times excruciating, is to be rated on the scale provided for injury of the nerve involved, with a maximum equal to severe, incomplete, paralysis. See nerve involved for diagnostic code number and rating. The maximum rating which may be assigned for neuritis not characterized by organic changes referred to in this section will be that for moderate, or with sciatic nerve involvement, for moderately severe, incomplete paralysis.” The sciatic clause is not the middle radicular group. On the DC 8511 scale, severe is 50 major and 40 minor, and moderate is 40 major and 30 minor. 38 CFR § 4.124 says: “Neuralgia, cranial or peripheral, characterized usually by a dull and intermittent pain, of typical distribution so as to identify the nerve, is to be rated on the same scale, with a maximum equal to moderate incomplete paralysis. See nerve involved for diagnostic code number and rating. Tic douloureux, or trifacial neuralgia, may be rated up to complete paralysis of the affected nerve.” The tic douloureux sentence is not this nerve. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. DC 8511’s lowest printed line is mild, 20 and 20.
DC 8510 is the upper radicular group. Its complete cell is also 70 and 60, and it says all shoulder and elbow movements lost or severely affected, hand and wrist movements not affected. That sentence is not the DC 8511 complete cell.
The 2026-10-01 text of 38 CFR § 3.309 does not contain “radicular.” DC 8511 is a rating-schedule line, not a § 3.309 name. “Other organic diseases of the nervous system” and “Peripheral neuropathy except where directly related to infectious causes” are family rows with no diagnostic code. This page does not assign DC 8511 to those names.

What the words mean in your records
Arm, elbow, and wrist
The complete cell says adduction, abduction and rotation of arm, flexion of elbow, and extension of wrist lost or severely affected. It does not say hand and wrist movements are not affected. That “not affected” sentence is DC 8510.
Major and Minor
Complete is 70 in the Major column and 60 in the Minor column. Severe is 50 and 40. Moderate is 40 and 30. Mild is 20 and 20.
Neuritis and neuralgia have no percentage on the row
8611 prints “Neuritis.” and 8711 prints “Neuralgia.” The caps are in § 4.123 and § 4.124, applied to this nerve’s scale.
Evidence that speaks to each level
70 major, 60 minor
What the rater needs to see. Complete paralysis of the middle radicular group: adduction, abduction and rotation of the arm, flexion of the elbow, and extension of the wrist lost or severely affected, and which side the Major or Minor column is using.
Where that usually lives. The neurological examination of the arm, elbow, and wrist, and the evidence of which hand is dominant.
50 and 40, 40 and 30, or 20 and 20
What the rater needs to see. Severe, moderate, or mild incomplete paralysis on this scale.
Where that usually lives. The same examination. Wholly sensory involvement is mild, or at most moderate, under the peripheral-nerve sentence.
VA’s public DBQ list, checked October 5, 2026, names “Peripheral Nerves” under Neurological. The file is Peripheral Nerves.
The exam form
DC 8511 prints 70 and 60, 50 and 40, 40 and 30, and 20 and 20. This page does not say the form replaces those columns.
Related codes and how they interact
The upper radicular group is a different complete sentence.
DC 8510’s complete cell says all shoulder and elbow movements lost or severely affected, hand and wrist movements not affected, also at 70 and 60. DC 8511’s complete cell names adduction, abduction and rotation of the arm, flexion of the elbow, and extension of the wrist.
Secondary conditions and presumptions
Section 3.309 does not contain “radicular” and does not assign DC 8511. This page does not add DC 8511 to a presumptive row.
A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
What is the VA rating for middle radicular group paralysis?
Diagnostic Code 8511 in 38 CFR § 4.124a uses Major and Minor columns. Complete is 70 and 60. Severe is 50 and 40. Moderate is 40 and 30. Mild is 20 and 20. Education only. VA decides on the evidence.
Which hand is major?
The column headings are Major and Minor. Section 4.69 says only one hand shall be considered dominant, determined by the evidence of record or by testing on VA examination. It does not use the words Major and Minor.
FAQ
What are the major and minor ratings for DC 8511?
Complete is 70 and 60. Severe is 50 and 40. Moderate is 40 and 30. Mild is 20 and 20. The complete cell names adduction, abduction and rotation of the arm, flexion of the elbow, and extension of the wrist. Education only.
Is this the upper radicular group?
No. The heading is “Middle radicular group.” The upper radicular heading, DC 8510, adds “(fifth and sixth cervicals).”
Do neuritis and neuralgia have their own percentages?
The rows 8611 and 8711 print “Neuritis.” and “Neuralgia.” and no percentage. Section 4.123 caps neuritis at severe incomplete paralysis when the organic changes that section names are present, and otherwise at moderate. Section 4.124 caps neuralgia at moderate incomplete paralysis. On this scale, severe is 50 major and 40 minor, and moderate is 40 major and 30 minor.
