Neoplasm, malignant, any specified part of the endocrine system — VA Diagnostic Code 7914

Endocrine

Diagnostic code

7914

38 CFR § 4.119 lists Diagnostic Code 7914 as “Neoplasm, malignant, any specified part of the endocrine system.” The title does not end with a colon. The rating on that same row is 100. A note follows. Appendix B to Part 4 indexes “Malignant neoplasm.” The catalog name is “Malignant Neoplasm.”

Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.119 source note prints 61 FR 20446, May 7, 1996, as amended at 82 FR 50804, Nov. 2, 2017.

The rating criteria at each level

7914 Neoplasm, malignant, any specified part of the endocrine system

Rating %: 100

Criteria (verbatim). Neoplasm, malignant, any specified part of the endocrine system

Note: A rating of 100 percent shall continue beyond the cessation of any surgical, X-ray, antineoplastic chemotherapy or other therapeutic procedure. Six months after discontinuance of such treatment, the appropriate disability rating shall be determined by mandatory VA examination. Any change in evaluation based upon that or any subsequent examination shall be subject to the provisions of § 3.105(e) of this chapter. If there has been no local recurrence or metastasis, rate on residuals.

The note says “X-ray, antineoplastic chemotherapy or other therapeutic procedure.” There is no comma before “or.” It says “100 percent” with a space, not the hyphenated “100-percent” used in DC 7904 Note (2).

The 100 percent evaluation is on the title row. The note continues that rating beyond the cessation of any surgical, X-ray, antineoplastic chemotherapy or other therapeutic procedure. Six months after discontinuance of such treatment, a mandatory VA examination determines the rating. Any change based upon that or any subsequent examination is subject to 38 CFR § 3.105(e). If there has been no local recurrence or metastasis, rate on residuals. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. DC 7914 does not print a 0 percent line. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.

DC 7915 is “Neoplasm, benign, any specified part of the endocrine system:”. The title ends with a colon. The rating cell is empty. The next cell says “Rate as residuals of endocrine dysfunction.” That sentence is not on DC 7914. Appendix B indexes DC 7915 as “Benign neoplasm.”

The 2026-10-01 text of 38 CFR § 3.309 lists “Tumors, malignant, or of the brain or spinal cord or peripheral nerves” and “Endocrinopathies.” Neither entry assigns DC 7914. 38 CFR § 3.311(b)(2) defines radiogenic disease and does not assign DC 7914. A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

What the words mean in your records

100 sits on the title row

The criteria cell and the 100 percent cell are the same row. The title does not end with a colon. Appendix B shortens the index entry to “Malignant neoplasm.”

Six months, then § 3.105(e)

The note names surgical treatment, X-ray, antineoplastic chemotherapy, or another therapeutic procedure. Six months after discontinuance, the rating comes from a mandatory VA examination. A change based on that examination, or a later one, is subject to § 3.105(e).

Residuals only if there is no local recurrence or metastasis

The last sentence of the note is “If there has been no local recurrence or metastasis, rate on residuals.” The note does not name the residual diagnostic code.

Evidence that speaks to each level

100 — on the title row, continued past treatment

What the rater needs to see. A malignant neoplasm of a specified part of the endocrine system, and the date treatment of the kinds the note names was discontinued, when the six-month period is the one being counted.

Where that usually lives. The diagnosis that specifies the part of the endocrine system, and the treatment record.

After the mandatory examination

What the rater needs to see. The examination six months after discontinuance. The note says a change based upon that or any subsequent examination is subject to § 3.105(e). If there has been no local recurrence or metastasis, rate on residuals.

Where that usually lives. The examination report and the decision that cites the residual code when that last sentence is the one being used.

VA’s public DBQ list, checked October 5, 2026, names both “Thyroid and Parathyroid” and “Endocrine (Other than Thyroid, Parathyroid or Diabetes Mellitus)” under Endocrinological. DC 7914 says “any specified part of the endocrine system,” so this page does not pick one file for every part. The files are Thyroid and Parathyroid and Endocrine (Other than Thyroid, Parathyroid or Diabetes Mellitus).

The exam form

DC 7914 prints 100 and one note. This page does not say either form replaces that note.

Related codes and how they interact

A benign endocrine neoplasm is DC 7915.

DC 7915 says to rate as residuals of endocrine dysfunction and does not print 100. DC 7914 prints 100 on the title row and uses the six-month note.

Secondary conditions and presumptions

Section 3.309 does not assign DC 7914. Section 3.311(b)(2) does not assign DC 7914. The schedule title is “any specified part of the endocrine system.”

A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

Short answers

How does VA rate a malignant endocrine neoplasm under DC 7914?

Under 38 CFR § 4.119, Diagnostic Code 7914 is 100 percent. The note continues that rating beyond the cessation of the treatment it names. Six months after discontinuance, a mandatory VA examination determines the rating, and a change is subject to § 3.105(e). If there has been no local recurrence or metastasis, rate on residuals. Education only. VA decides on the evidence.

Is a benign endocrine neoplasm rated on this row?

DC 7915 is the benign row. It says to rate as residuals of endocrine dysfunction. DC 7914 is the malignant row, at 100, with the note quoted above.

FAQ

Does the DC 7914 title end with a colon?

No. The row is “Neoplasm, malignant, any specified part of the endocrine system” at 100. DC 7915’s benign title ends with a colon. Education only.

What treatment does the note name?

Any surgical, X-ray, antineoplastic chemotherapy or other therapeutic procedure. There is no comma before “or.” Six months after discontinuance, a mandatory VA examination determines the rating.

What does Appendix B call this code?

“Malignant neoplasm.” The rating table’s longer title names any specified part of the endocrine system. The catalog row says “Malignant Neoplasm.”
