38 CFR Part 4 — 38 CFR § 4.119
Acromegaly — VA Diagnostic Code 7908
Endocrine
Diagnostic code
7908
38 CFR § 4.119 lists Diagnostic Code 7908 as “Acromegaly:”. The lines are 100, 60, and 30. Appendix B to Part 4 says “Acromegaly.” No note is printed between DC 7908 and DC 7909.
Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.119 source note prints 61 FR 20446, May 7, 1996, as amended at 82 FR 50804, Nov. 2, 2017.
The rating criteria at each level
7908 Acromegaly
Rating %: 100
Criteria (verbatim). Evidence of increased intracranial pressure (such as visual field defect), arthropathy, glucose intolerance, and either hypertension or cardiomegaly
Rating %: 60
Criteria (verbatim). Arthropathy, glucose intolerance, and hypertension
Rating %: 30
Criteria (verbatim). Enlargement of acral parts or overgrowth of long bones
| Rating % | Criteria (verbatim) |
|---|---|
| 100 | Evidence of increased intracranial pressure (such as visual field defect), arthropathy, glucose intolerance, and either hypertension or cardiomegaly |
| 60 | Arthropathy, glucose intolerance, and hypertension |
| 30 | Enlargement of acral parts or overgrowth of long bones |
The 100 percent line joins four findings with commas and a final “and.” The parenthetical “such as visual field defect” sits inside the increased-intracranial-pressure finding. “Either hypertension or cardiomegaly” is the last finding on that line.
The 60 percent line names arthropathy, glucose intolerance, and hypertension. It does not name increased intracranial pressure or cardiomegaly. The 30 percent line uses “or” between enlargement of acral parts and overgrowth of long bones.
The block does not say to evaluate hypertension, cardiomegaly, or a visual field defect under another diagnostic code. DC 7101 is hypertensive vascular disease. Its public page is the hypertension guide. The DC 7908 lines use the word “hypertension” as one finding in a combination. They do not say “separately evaluate under DC 7101.” The words “glucose intolerance” are not the title “Diabetes mellitus.” 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.
DC 7908 does not print a 0 percent line. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. DC 7909, diabetes insipidus, is the next code. Its 30 percent line and its note are not printed under DC 7908.

What the words mean in your records
The 100 percent combination
The line requires evidence of increased intracranial pressure (such as visual field defect), arthropathy, glucose intolerance, and either hypertension or cardiomegaly. “Such as” is the example inside the first finding.
The 60 percent line drops two findings
It names arthropathy, glucose intolerance, and hypertension. It does not name increased intracranial pressure, visual field defect, or cardiomegaly.
Acral parts or long bones
The 30 percent line is “Enlargement of acral parts or overgrowth of long bones.” The “or” is on that line. The line does not repeat the 100 or 60 percent findings.
Evidence that speaks to each level
100 or 60
What the rater needs to see. Each finding the line names: for 100, increased intracranial pressure (such as visual field defect), arthropathy, glucose intolerance, and either hypertension or cardiomegaly; for 60, arthropathy, glucose intolerance, and hypertension.
Where that usually lives. The endocrine examination and the records that name those findings.
30
What the rater needs to see. Enlargement of acral parts or overgrowth of long bones.
Where that usually lives. The examination that describes the acral parts or the long bones.
VA’s public DBQ list, checked October 5, 2026, names “Endocrine (Other than Thyroid, Parathyroid or Diabetes Mellitus)” under Endocrinological. The file is Endocrine (Other than Thyroid, Parathyroid or Diabetes Mellitus).
The exam form
DC 7908 prints three percentage lines. This page does not say the form replaces those lines.
Related codes and how they interact
Hypertension on this line is not an instruction to use DC 7101.
The hypertension guide covers DC 7101. DC 7908 includes the word hypertension in the 100 and 60 percent combinations. The acromegaly block does not say to rate that finding under DC 7101.
Secondary conditions and presumptions
The 2026-10-01 text of 38 CFR § 3.309 does not contain “acromegaly.” DC 7908 is not a chronic-disease row in that section. 38 CFR § 3.320 does not contain “acromegaly.”
Short answers
What is the VA rating for acromegaly?
Under 38 CFR § 4.119, Diagnostic Code 7908 prints 100, 60, and 30. It does not print a 0 percent line. Education only. VA decides on the evidence.
Does this code tell VA to rate hypertension separately?
The DC 7908 lines do not say that. They use hypertension as one finding in the 100 and 60 percent combinations. Hypertensive vascular disease is DC 7101.
FAQ
What percentages does DC 7908 print?
100, 60, and 30. The 100 percent line requires increased intracranial pressure (such as visual field defect), arthropathy, glucose intolerance, and either hypertension or cardiomegaly. The 60 percent line is arthropathy, glucose intolerance, and hypertension. The 30 percent line is enlargement of acral parts or overgrowth of long bones. Education only.
Does the 60 percent line include cardiomegaly?
No. Cardiomegaly appears only in the 100 percent line, as the alternative to hypertension. The 60 percent line names hypertension, not cardiomegaly.
Is acromegaly named as a presumptive disease?
Section 3.309 does not contain “acromegaly.” Section 3.320 does not contain it either.
