38 CFR Part 4 — 38 CFR § 4.115b
Nephrosclerosis, arteriolar — VA Diagnostic Code 7507
Genitourinary
Diagnostic code
7507
38 CFR § 4.115b lists Diagnostic Code 7507 as “Nephrosclerosis, arteriolar:”. The title ends with a colon. The rating cell is empty. The next cell names renal dysfunction, hypertension or heart disease, and then an elevation rule for a rating under the cardiovascular schedule. That cell ends with a period. There is no comma before “or.” Appendix B to Part 4 indexes “Nephrosclerosis, arteriolar.” with a period and without the colon.

The rating criteria at each level
7507 Nephrosclerosis, arteriolar — the cell this code prints
Rating %: —
Criteria (verbatim). Rate according to predominant symptoms as renal dysfunction, hypertension or heart disease. If rated under the cardiovascular schedule, however, the percentage rating which would otherwise be assigned will be elevated to the next higher evaluation.

Note: GFR, estimated GFR (eGFR), and creatinine-based approximations of GFR will be accepted for evaluation purposes under this section when determined to be appropriate and calculated by a medical professional.
The 2 in m² is a superscript in § 4.115a. The DC 7507 cell does not print those GFR lines. It names “renal dysfunction.”
The elevation sentence does not print the next number. It says the percentage rating which would otherwise be assigned will be elevated to the next higher evaluation, if rated under the cardiovascular schedule.
The cell says “hypertension or heart disease.” It does not print “DC 7101,” and it does not print a heart-formula diagnostic code.
38 CFR § 4.115a says: “Where diagnostic codes refer the decision maker to these specific areas of dysfunction, only the predominant area of dysfunction shall be considered for rating purposes.” It also says distinct disabilities may be evaluated separately under that section, pursuant to § 4.14, if the symptoms do not overlap. The DC 7507 cell says “predominant symptoms,” and it names renal dysfunction, hypertension, or heart disease. Hypertension and heart disease are not headings in § 4.115a. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.
DC 7530 says only “Rate as renal dysfunction.” It does not print the elevation sentence. DC 7531 is kidney transplant, with a minimum rating of 30 thereafter. That minimum is not a DC 7507 line. Hypertensive vascular disease has its own public page, DC 7101. The general rating formula for diseases of the heart is discussed on the DC 7005 page. DC 7507 does not say to use either of those codes by number.
The 2026-10-01 text of 38 CFR § 3.309 does not contain “nephrosclerosis.” It says “Cardiovascular-renal disease, including hypertension.” The parenthetical says this term applies to combination involvement of the type of arteriosclerosis, nephritis, and organic heart disease. That row has no diagnostic code. It does not assign DC 7507.

What the words mean in your records
Three named areas, one cell
The cell says to rate according to predominant symptoms as renal dysfunction, hypertension or heart disease. There is no comma before “or.”
Next higher evaluation
If the rating is under the cardiovascular schedule, the percentage which would otherwise be assigned is elevated to the next higher evaluation. The cell does not list that next number.
Renal dysfunction is the area with printed percentages
The 100, 80, 60, 30, and 0 lines live under § 4.115a. The 100 percent line says “requiring regular routine dialysis.” DC 7507 does not use those words in its own cell.
Evidence that speaks to each level
The DC 7507 cell
What the rater needs to see. Which of the three named symptom areas is the predominant one, and, if the rating is under the cardiovascular schedule, the elevation the second sentence states.
Where that usually lives. The decision and the findings it uses for that choice.
Renal dysfunction, when that is the area used
What the rater needs to see. The GFR range for at least 3 consecutive months during the past 12 months, regular routine dialysis, eligible transplant status, or the casts, structural abnormalities, or ACR the 0 percent line names.
Where that usually lives. Laboratory results calculated by a medical professional, as the renal note states.
VA’s public DBQ list, checked October 5, 2026, names “Kidney Conditions” under Genitourinary. The file is Kidney Conditions.
The exam form
DC 7507 prints the predominant-symptoms sentence and no percentage of its own. This page does not say the form replaces that sentence.
Related codes and how they interact
Dialysis disease and transplant are different sentences.
DC 7530 is only “Rate as renal dysfunction.” DC 7531 has a post-transplant minimum of 30. Neither prints the DC 7507 elevation sentence.
Secondary conditions and presumptions
Section 3.309 does not contain “nephrosclerosis,” and it does not assign DC 7507. Cardiovascular-renal disease, including hypertension, stays a family row without a diagnostic code.
A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
How does VA rate arteriolar nephrosclerosis?
Under 38 CFR § 4.115b, Diagnostic Code 7507 rates according to predominant symptoms as renal dysfunction, hypertension or heart disease, and it elevates a cardiovascular-schedule percentage to the next higher evaluation. Renal dysfunction percentages are in § 4.115a. Education only. VA decides on the evidence.
Is nephrosclerosis the cardiovascular-renal chronic-disease row?
Section 3.309(a) says “Cardiovascular-renal disease, including hypertension,” and it describes combination involvement of the type of arteriosclerosis, nephritis, and organic heart disease. That row has no diagnostic code. The 2026-10-01 text does not contain “nephrosclerosis,” and it does not assign DC 7507.
FAQ
What does DC 7507 tell the rater to use?
Rate according to predominant symptoms as renal dysfunction, hypertension or heart disease. If rated under the cardiovascular schedule, elevate the percentage which would otherwise be assigned to the next higher evaluation. The cell ends with a period and does not print that next number. Education only.
Where are the renal percentages?
Under § 4.115a, Renal dysfunction: 100, 80, 60, 30, and 0. The renal note accepts GFR, estimated GFR, and creatinine-based approximations when a medical professional calculates them and they are appropriate. The 2 in m² is a superscript.
Does the cell name DC 7101?
No. It says “hypertension.” It does not print a diagnostic-code number for hypertension or for heart disease.
