What the words mean in your records
Extension limited to
Each line uses those words plus a degree: 45, 30, 20, 15, 10, or 5. The schedule does not define extension in a sentence under this code. A larger degree on this list is a higher percentage.
How extension is measured
38 CFR § 4.46 (Accurate measurement) says the use of a goniometer in the measurement of limitation of motion is indispensable in examinations conducted within the Department of Veterans Affairs. 38 CFR § 4.59 (Painful motion) says the joints involved should be tested for pain on both active and passive motion, in weight-bearing and nonweight-bearing and, if possible, with the range of the opposite undamaged joint. The same section says the intent is to recognize actually painful, unstable, or malaligned joints, due to healed injury, as entitled to at least the minimum compensable rating for the joint. 38 CFR § 4.40 (Functional loss) says functional loss may be due to pain, supported by adequate pathology and evidenced by the visible behavior of the claimant undertaking the motion, and that weakness is as important as limitation of motion. 38 CFR § 4.45 (The joints) directs inquiry to less movement than normal, more movement than normal, weakened movement, excess fatigability, incoordination, and pain on movement, swelling, deformity, or atrophy of disuse. For arthritis, it names the shoulder, elbow, wrist, hip, knee, and ankle as major joints. 38 CFR § 4.71 says Plates I and II provide a standardized description of ankylosis and joint motion measurement. This page does not retype the plates.
Flexion of the same knee
VA General Counsel Precedent Opinion VAOPGCPREC 9-2004 (September 17, 2004), published on VA.gov, holds: “Separate ratings under DC 5260 (leg, limitation of flexion) and DC 5261 (leg, limitation of extension), both currently codified at 38 C.F.R. § 4.71a, may be assigned for disability of the same joint.” 38 CFR § 4.14 still says the evaluation of the same disability under various diagnoses is to be avoided. The opinion and the regulation are both in the record a rater uses. This page does not apply them to a particular knee.
Evidence that speaks to each level
50 — extension limited to 45°
What the rater needs to see. Extension limited to 45 degrees.
Where that usually lives. Goniometer measurement in a VA examination under § 4.46.
30 — extension limited to 20°
What the rater needs to see. Extension limited to 20 degrees. The 30 percent line is 20 degrees. Flexion limited to 30 degrees is the DC 5260 20 percent line, a different code and a different motion.
Where that usually lives. The same examination, recorded as extension rather than flexion.
10 — extension limited to 10°
What the rater needs to see. Extension limited to 10 degrees.
Where that usually lives. Active and passive testing, and weight-bearing and nonweight-bearing when § 4.59 applies.
0 — extension limited to 5°
What the rater needs to see. The printed 0 percent line is extension limited to 5 degrees.
Where that usually lives. The same measurements.
VA’s public DBQ list names “Knee and Lower Leg.” This site’s page is the knee DBQ guide. The list is the public DBQ list.
The exam form
The public DBQ list, checked October 3, 2026, names “Knee and Lower Leg” under Musculoskeletal. It is a public form.
Section 4.46 requires a goniometer for limitation of motion in a VA examination. Section 4.59 requires pain testing on active and passive motion, in weight-bearing and nonweight-bearing, and, if possible, with the opposite undamaged joint.
Related codes and how they interact
Flexion, ankylosis, and replacement
DC 5260 is limitation of flexion: 15 degrees is 30 percent, 30 degrees is 20 percent, 45 degrees is 10 percent, and 60 degrees is 0 percent.
DC 5256 rates ankylosis of the knee. The knee guide is the public page.
DC 5055 says intermediate degrees of residual weakness, pain, or limitation of motion after prosthetic replacement are rated by analogy to diagnostic codes 5256, 5261, or 5262. The minimum evaluation for total replacement only is 30. The public page is the knee replacement guide.
38 CFR § 4.71a Diagnostic Code 5003, “Degenerative arthritis, other than post-traumatic,” says degenerative arthritis established by X-ray findings is rated on limitation of motion under the appropriate diagnostic codes for the specific joint or joints involved. When that limitation is noncompensable under those codes, “a rating of 10 pct is for application for each such major joint or group of minor joints affected by limitation of motion, to be combined, not added under diagnostic code 5003.” Limitation of motion must be objectively confirmed by findings such as swelling, muscle spasm, or satisfactory evidence of painful motion. In the absence of limitation of motion, the code prints 20 for X-ray evidence of involvement of 2 or more major joints or 2 or more minor joint groups, with occasional incapacitating exacerbations, and 10 for that X-ray involvement without the exacerbations line. Note (1): those 20 pct and 10 pct ratings based on X-ray findings will not be combined with ratings based on limitation of motion. DC 5010, “Post-traumatic arthritis,” says: “Rate as limitation of motion, dislocation, or other specified instability under the affected joint. If there are 2 or more joints affected, each rating shall be combined in accordance with § 4.25.”
Secondary conditions and presumptions
No secondary condition is named in DC 5261. 38 CFR § 3.309 (Disease subject to presumptive service connection) lists “Arthritis” among the chronic diseases. It does not name this diagnostic code. The chronic-disease rule uses the time limits in 38 CFR § 3.307. Paragraph (a)(3) says the disease must have become manifest to a degree of 10 percent or more within 1 year (for Hansen's disease (leprosy) and tuberculosis, within 3 years; multiple sclerosis, within 7 years) from the date of separation from service. Any secondary question that is not written into the schedule is a medical question on the facts under 38 CFR § 3.310 (Disabilities that are proximately due to, or aggravated by, service-connected disease or injury). Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
What is the VA rating for limitation of extension of the leg?
Under 38 CFR § 4.71a, Diagnostic Code 5261, extension limited to 45 degrees is 50 percent, to 30 degrees is 40 percent, to 20 degrees is 30 percent, to 15 degrees is 20 percent, to 10 degrees is 10 percent, and to 5 degrees is 0 percent. Education only. VA decides on the evidence.
Which examination finding matches DC 5261?
A goniometer measurement of extension, under § 4.46, tested for pain on active and passive motion under § 4.59. Flexion measurements are the DC 5260 lines.
FAQ
How does VA rate limitation of knee extension under DC 5261?
Under 38 CFR § 4.71a, extension limited to 45 degrees is 50 percent, to 30 degrees is 40 percent, to 20 degrees is 30 percent, to 15 degrees is 20 percent, to 10 degrees is 10 percent, and to 5 degrees is 0 percent. Education only.
Is a 10 degree extension limit the same as a 10 percent flexion rating?
No. Extension limited to 10 degrees is the DC 5261 10 percent line. Flexion limited to 45 degrees is the DC 5260 10 percent line. They are different motions and different codes.
Can the same knee be rated for both motions?
VAOPGCPREC 9-2004 holds that separate ratings under DC 5260 and DC 5261 may be assigned for disability of the same joint. Section 4.14 says the evaluation of the same disability under various diagnoses is to be avoided.
