Little finger, amputation of — VA Diagnostic Code 5156

Musculoskeletal

Diagnostic code

5156

38 CFR § 4.71a lists Diagnostic Code 5156 under a centered line that reads “single finger amputations,” in lowercase, inside the table captioned “Amputations: Upper Extremity.” The title is “Little finger, amputation of:”. The colon is in the schedule. The rating cells on that title are empty. “With metacarpal resection (more than one-half the bone lost)” is 20 in the Major column and 20 in the Minor column. “Without metacarpal resection, at proximal interphalangeal joint or proximal thereto” is 10 in the Major column and 10 in the Minor column. Neither number carries a superscript. Appendix B to Part 4 indexes the code as “Little finger.”

Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.71a source note prints 29 FR 6718, May 22, 1964, as amended at 34 FR 5062, Mar. 11, 1969; 40 FR 42536, Sept. 15, 1975; 41 FR 11294, Mar. 18, 1976; 43 FR 45350, Oct. 2, 1978; 51 FR 6411, Feb. 24, 1986; 61 FR 20439, May 7, 1996; 67 FR 48785, July 26, 2002; 67 FR 54349, Aug. 22, 2002; 68 FR 51456, Aug. 27, 2003; 69 FR 32450, June 10, 2004; 80 FR 42041, July 16, 2015; 85 FR 76460, Nov. 30, 2020, 85 FR 85523, Dec. 29, 2020, 86 FR 8142, Feb. 4, 2021. The § 4.63 source note prints 29 FR 6718, May 22, 1964, as amended at 43 FR 45349, Oct. 2, 1978.

The rating criteria at each level

5156 Little finger, amputation of — Major and Minor columns

Rating %: 20 major, 20 minor

Criteria (verbatim). With metacarpal resection (more than one-half the bone lost)

Rating %: 10 major, 10 minor

Criteria (verbatim). Without metacarpal resection, at proximal interphalangeal joint or proximal thereto

The table is headed Amputations: Upper Extremity, with columns Rating, Major, and Minor. The 20 and the 10 do not have a superscript.

Footnote 1 of this table reads: Entitled to special monthly compensation. That footnote is printed on the rows that carry superscript 1. It is not printed on the 20 or the 10 of DC 5156. The footnote does not say “Also.”

The next row after the 10 is a note: “The single finger amputation ratings are the only applicable ratings for amputations of whole or part of single fingers.” The note ends with a period. It is printed after the little-finger lines and speaks to single finger amputations.

DC 5156 does not print a “Through middle phalanx or at distal joint” line. The index-finger code, DC 5153, does print that line, at 10 major and 10 minor.

38 CFR § 4.69 says handedness for the purpose of a dominant rating will be determined by the evidence of record, or by testing on VA examination. Only one hand shall be considered dominant. The injured hand, or the most severely injured hand, of an ambidextrous individual will be considered the dominant hand for rating purposes. Section 4.69 does not use the words “Major” and “Minor.” Those are the column headings. On DC 5156 the two columns print the same number on each line, 20 and 20, then 10 and 10.

Paragraphs (a) through (f) are printed in the multiple-finger block, before the centered line “single finger amputations.” Paragraph (b) says amputation through middle phalanges will be rated as prescribed for unfavorable ankylosis of the fingers. Paragraph (c) says amputations at distal joints, or through distal phalanges, other than negligible losses, will be rated as prescribed for favorable ankylosis of the fingers. Paragraph (d) adds 10 percent, not combined, for metacarpal resection of more than one-half the bone in multiple fingers injuries, subject to the amputation rule applied to the forearm. The note after DC 5156 says the single finger amputation ratings are the only applicable ratings for amputations of whole or part of single fingers. This page quotes both texts. It does not choose between them.

DC 5155 is “Ring finger, amputation of:”. It prints the same two lines and the same 20 and 10 in both columns. DC 5227 is ring or little finger, ankylosis of: unfavorable or favorable is 0 major and 0 minor. Paragraph (3)(i) of the digit rules says that if both the metacarpophalangeal and proximal interphalangeal joints of a digit are ankylosed, and either is in extension or full flexion, or there is rotation or angulation of a bone, evaluate as amputation without metacarpal resection, at the proximal interphalangeal joint or proximal thereto. That sentence names the 10 percent line. It does not name the 20 percent metacarpal-resection line. DC 5230 is ring or little finger, limitation of motion: any limitation of motion is 0 major and 0 minor.

38 CFR § 4.68 says the combined rating for disabilities of an extremity shall not exceed the rating for the amputation at the elective level, were amputation to be performed. The example is the 40 percent evaluation, diagnostic code 5165. The example does not name DC 5156. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.

The code prints no 0 percent line. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. 38 CFR § 3.350(a) says special monthly compensation under 38 U.S.C. 1114(k) is payable for each anatomical loss or loss of use of one hand, one foot, and the other losses that sentence lists. The DC 5156 cells do not carry footnote 1. This page does not state a payment amount.

The 2026-10-01 text of 38 CFR § 3.309 does not contain “little finger.” DC 5156 is a rating-schedule line.

What the words mean in your records

20 and 20, then 10 and 10

Metacarpal resection of more than one-half the bone is 20 in each column. Without that resection, at the proximal interphalangeal joint or proximal thereto, is 10 in each column. The title row itself has empty rating cells.

No superscript on these numbers

Footnote 1, “Entitled to special monthly compensation,” belongs to the rows that print superscript 1. Five digits of one hand is one of those rows. The little-finger 20 and 10 do not print that superscript.

Appendix B drops the amputation words

The rating title is “Little finger, amputation of:”. Appendix B says “Little finger.” and ends with a period.

Evidence that speaks to each level

20 major, 20 minor

What the rater needs to see. Metacarpal resection, more than one-half the bone lost, which is the first DC 5156 line.

Where that usually lives. The amputation record, including how much of the metacarpal was removed. Section 4.69 is how dominance is determined.

10 major, 10 minor

What the rater needs to see. Amputation without metacarpal resection, at the proximal interphalangeal joint or proximal thereto.

Where that usually lives. The same operative or examination record, read against that line.

VA’s public DBQ list, checked October 5, 2026, names “Amputations” and “Hand and Finger” under Musculoskeletal. The files are Amputations and Hand and Finger.

The exam form

DC 5156 prints 20 and 20, then 10 and 10, with no superscript. This page does not say either form replaces those rows.

Related codes and how they interact

The ring finger uses the same two lines.

DC 5155 prints 20 and 20 with metacarpal resection and 10 and 10 without it. DC 5153, the index finger, adds a third line, through the middle phalanx or at the distal joint, at 10 and 10. DC 5156 does not print that third line.

Secondary conditions and presumptions

Section 3.309 does not contain “little finger.” DC 5156 is a rating-schedule line.

A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

Short answers

How does VA rate amputation of the little finger?

Under 38 CFR § 4.71a, Diagnostic Code 5156 is 20 percent for the major hand and 20 percent for the minor hand when more than one-half of the metacarpal is resected, and 10 percent in each column without that resection, at the proximal interphalangeal joint or proximal thereto. Education only. VA decides on the evidence.

Does Appendix B use the rating-table title?

The rating title is “Little finger, amputation of:”. Appendix B indexes “Little finger.” with a period.

FAQ

What are the ratings for little-finger amputation?

With metacarpal resection of more than one-half the bone, 20 in the Major column and 20 in the Minor column. Without metacarpal resection, at the proximal interphalangeal joint or proximal thereto, 10 and 10. Neither number has a superscript. Education only.

Does the little finger have a middle-phalanx line?

DC 5156 prints two lines. It does not print “Through middle phalanx or at distal joint.” That third line is on the index finger, DC 5153. The note after the little-finger lines says the single finger amputation ratings are the only applicable ratings for amputations of whole or part of single fingers.

Is ankylosis of the little finger the same code?

Ankylosis of the ring or little finger is DC 5227. Unfavorable or favorable is 0 and 0. Paragraph (3)(i) can point a specified ankylosis to the without-metacarpal-resection amputation evaluation. That sentence does not name the 20 percent metacarpal-resection line.
