Complete amputation, upper extremity — VA Diagnostic Code 5120

Musculoskeletal

Diagnostic code

5120

38 CFR § 4.71a lists Diagnostic Code 5120 in the table captioned “Amputations: Upper Extremity,” under the heading “Arm, amputation of:”. The code line is “Complete amputation, upper extremity:”. That line ends with a colon, and its rating cells are empty. Two rows sit under it and do not have their own diagnostic codes. Forequarter amputation is 100 in the Major column and 100 in the Minor column. Disarticulation is 90 and 90. Footnote 1 is on all four numbers. Neither row ends with a period. Appendix B to Part 4 indexes “Complete amputation, upper extremity.” and does end with a period. The catalog slug is dc-5120-complete-amputation-upper-extremity.

The rating criteria at each level

5120 Complete amputation, upper extremity — Major and Minor columns

Rating %: 100 major, 100 minor

Criteria (verbatim). Forequarter amputation (involving complete removal of the humerus along with any portion of the scapula, clavicle, and/or ribs)

Rating %: 90 major, 90 minor

Criteria (verbatim). Disarticulation (involving complete removal of the humerus only)

The heading above DC 5120 is “Arm, amputation of:”. The colon on “Complete amputation, upper extremity:” is on the code line. The two rows under it are not separately numbered.

Forequarter amputation says “complete removal of the humerus along with any portion of the scapula, clavicle, and/or ribs”. Disarticulation says “complete removal of the humerus only”. The word “only” is in that second row.

Footnote 1 reads: Entitled to special monthly compensation. It does not say “Also,” does not name § 3.350, and does not name an SMC level. The lower-extremity amputation footnote does say “Also.” That is a different footnote.

Appendix B indexes the code line only. It does not repeat the forequarter or disarticulation rows.

38 CFR § 4.69 says handedness for the purpose of a dominant rating will be determined by the evidence of record, or by testing on VA examination. Only one hand shall be considered dominant. The injured hand, or the most severely injured hand, of an ambidextrous individual will be considered the dominant hand for rating purposes. Section 4.69 does not use the words “Major” and “Minor.” Those are the column headings.

DC 5121 is the next numbered row: “Above insertion of deltoid,” 90 major and 80 minor, with the same footnote 1. The DC 5120 disarticulation row is 90 and 90. The DC 5121 minor number is 80, not 90. The multiple-finger paragraphs (a) through (f) are printed under the “multiple finger amputations” line, which is below DC 5125. They are not printed on DC 5120.

38 CFR § 4.68 says the combined rating for disabilities of an extremity shall not exceed the rating for the amputation at the elective level, were amputation to be performed. For example, the combined evaluations for disabilities below the knee shall not exceed the 40 percent evaluation, diagnostic code 5165. This 40 percent rating may be further combined with evaluation for disabilities above the knee but not to exceed the above the knee amputation elective level. Painful neuroma of a stump after amputation shall be assigned the evaluation for the elective site of reamputation. The example names diagnostic code 5165. It does not name DC 5120. 38 CFR § 4.63 defines loss of use of a hand or a foot for special monthly compensation. Section 4.63 does not name forequarter amputation and does not name DC 5120.

38 CFR § 3.350(a) says special monthly compensation under 38 U.S.C. 1114(k) is payable for each anatomical loss or loss of use of one hand, one foot, and the other losses that sentence lists. The DC 5120 footnote does not cite § 3.350. This page does not state a payment amount. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just. The code prints no 0 percent line. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met.

The 2026-10-01 text of 38 CFR § 3.309 paragraph (c) says ischemic heart disease or other cardiovascular disease developing in a veteran who has a service-connected amputation of one lower extremity at or above the knee, or service-connected amputations of both lower extremities at or above the ankles, shall be held to be the proximate result of the service-connected amputation or amputations. That paragraph does not assign DC 5120, DC 5130, or DC 5131, and it does not say upper extremity, humerus, or finger. This page does not add this code to that row. A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

What the words mean in your records

Two rows, one code

Forequarter is 100 and 100. Disarticulation is 90 and 90. Both carry footnote 1. Neither row has a diagnostic code of its own.

“and/or ribs” is only on the forequarter row

That row includes any portion of the scapula, clavicle, and/or ribs, with complete removal of the humerus. Disarticulation stops at complete removal of the humerus only.

The footnote does not say “Also”

Footnote 1 of the upper-extremity amputation table reads “Entitled to special monthly compensation.” The lower-extremity table uses the word “Also.”

Evidence that speaks to each level

100 major, 100 minor

What the rater needs to see. Forequarter amputation involving complete removal of the humerus along with any portion of the scapula, clavicle, and/or ribs.

Where that usually lives. The amputation record. Section 4.69 is how dominance is determined. Both columns on this row are 100.

90 major, 90 minor

What the rater needs to see. Disarticulation involving complete removal of the humerus only.

Where that usually lives. The same record. The word “only” is the schedule’s contrast with the forequarter row.

VA’s public DBQ list, checked October 5, 2026, names “Amputations” under Musculoskeletal. The file is Amputations.

The exam form

DC 5120 prints 100 major and 100 minor for forequarter amputation, and 90 major and 90 minor for disarticulation, with footnote 1. This page does not say the form replaces those rows.

Related codes and how they interact

Above the deltoid is the next number.

DC 5121 is 90 major and 80 minor. It is not one of the two unnumbered rows under DC 5120.

Secondary conditions and presumptions

Short answers

How does VA rate complete amputation of the upper extremity?

Under 38 CFR § 4.71a, Diagnostic Code 5120 is 100 and 100 for forequarter amputation and 90 and 90 for disarticulation, with footnote 1 on each number. Education only. VA decides on the evidence.

Is arm amputation above the deltoid the same 100?

No. Above insertion of the deltoid is DC 5121, at 90 major and 80 minor. The 100 pair is the forequarter row under DC 5120.

FAQ

What are the ratings for complete amputation of the upper extremity?

Forequarter amputation is 100 major and 100 minor. Disarticulation is 90 major and 90 minor. Footnote 1 says “Entitled to special monthly compensation.” It does not say “Also.” Education only. This page does not state a payment amount.

Do the two rows have separate diagnostic codes?

No. Both sit under Diagnostic Code 5120. Appendix B indexes “Complete amputation, upper extremity.” and does not list the two rows.

Does § 3.309 assign DC 5120?

No. Section 3.309(c) is about cardiovascular disease after a service-connected amputation of a lower extremity. It does not assign DC 5120.
