Eleventh (spinal accessory, external branch) cranial nerve paralysis — VA Diagnostic Code 8211

Neurological

Diagnostic code

8211

38 CFR § 4.124a lists “Eleventh (spinal accessory, external branch) cranial nerve.” The heading ends with a period. Diagnostic Code 8211 is “Paralysis of:”. Complete is 30. Incomplete, severe is 20. Incomplete, moderate is 10. The note depends upon loss of motor function of the sternomastoid and trapezius muscles. Appendix B to Part 4 indexes “Eleventh (spinal accessory, external branch), paralysis.” The catalog slug is dc-8211-eleventh-spinal-accessory-external-branch-paralysis.

The rating criteria at each level

8211 Paralysis of the eleventh (spinal accessory, external branch) cranial nerve

Rating %: 30

Criteria (verbatim). Complete

Rating %: 20

Criteria (verbatim). Incomplete, severe

Rating %: 10

Criteria (verbatim). Incomplete, moderate

Note: Dependent upon loss of motor function of sternomastoid and trapezius muscles.

The note ends with a period. In the 2026-10-01 table the word Note is bold and the colon is printed immediately after it. There is no space before the colon. DC 8210’s note prints a space before the colon. The three percentage lines do not end with a period.

There is no mild line. The following rows are “8311 Neuritis.” and “8411 Neuralgia.” This page does not move those rows onto DC 8211.

The Diseases of the Cranial Nerves block says: Disability from lesions of peripheral portions of first, second, third, fourth, sixth, and eighth nerves will be rated under the Organs of Special Sense. The ratings for the cranial nerves are for unilateral involvement; when bilateral, combine but without the bilateral factor. The eleventh nerve is not in the special-sense list. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just. 38 CFR § 4.14 says the evaluation of the same disability under various diagnoses is to be avoided, and that the evaluation of the same manifestation under different diagnoses is to be avoided.

38 CFR § 4.123 says: “Neuritis, cranial or peripheral, characterized by loss of reflexes, muscle atrophy, sensory disturbances, and constant pain, at times excruciating, is to be rated on the scale provided for injury of the nerve involved, with a maximum equal to severe, incomplete, paralysis. See nerve involved for diagnostic code number and rating. The maximum rating which may be assigned for neuritis not characterized by organic changes referred to in this section will be that for moderate, or with sciatic nerve involvement, for moderately severe, incomplete paralysis.” On this scale, severe incomplete paralysis is 20. Moderate is 10. There is no moderately severe line. The sciatic clause is a different nerve. DC 8311 prints “Neuritis.” and no percentage. 38 CFR § 4.124 says: “Neuralgia, cranial or peripheral, characterized usually by a dull and intermittent pain, of typical distribution so as to identify the nerve, is to be rated on the same scale, with a maximum equal to moderate incomplete paralysis. See nerve involved for diagnostic code number and rating. Tic douloureux, or trifacial neuralgia, may be rated up to complete paralysis of the affected nerve.” The tic douloureux sentence is not this nerve. DC 8411 prints “Neuralgia.” and no percentage. Moderate on this scale is 10.

The 2026-10-01 text of 38 CFR § 3.309 does not contain “spinal accessory,” “sternomastoid,” or “8211.” This page does not add DC 8211 to a presumptive row.

What the words mean in your records

Three levels

Complete is 30. Incomplete, severe is 20. Incomplete, moderate is 10. The table does not print a mild line under DC 8211.

The muscles in the note

Sternomastoid and trapezius. The note says the rating is dependent upon loss of motor function of those muscles. It ends with a period.

Neuritis and neuralgia are the next rows

DC 8311 is neuritis. DC 8411 is neuralgia. Their titles are separate from “Paralysis of.”

Evidence that speaks to each level

30, 20, or 10

What the rater needs to see. Whether paralysis is complete, incomplete and severe, or incomplete and moderate.

Where that usually lives. The cranial-nerve examination, including motor function of the sternomastoid and trapezius muscles.

VA’s public DBQ list, checked October 5, 2026, names “Cranial Nerve Conditions” under Neurological. The file is Cranial Nerve Conditions.

The exam form

DC 8211 prints 30, 20, and 10. This page does not say the form replaces those lines.

Related codes and how they interact

The same nerve has neuritis and neuralgia rows

After the paralysis note, the table lists 8311 Neuritis and 8411 Neuralgia. Those rows do not print a percentage. Neuralgia of this nerve is DC 8411.

Secondary conditions and presumptions

Section 3.309 does not contain “spinal accessory,” “sternomastoid,” or “8211.” This page does not add DC 8211 to a presumptive row.

A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

Short answers

How does VA rate eleventh cranial nerve paralysis under DC 8211?

Under 38 CFR § 4.124a, Diagnostic Code 8211 is 30 percent for complete paralysis, 20 percent for incomplete severe paralysis, and 10 percent for incomplete moderate paralysis. The note names the sternomastoid and trapezius muscles. Education only. VA decides on the evidence.

Is the colon spaced like the DC 8210 note?

No. The DC 8211 table prints the colon immediately after the bold word Note. The DC 8210 note prints a space before the colon.

FAQ

What are the DC 8211 percentages?

Complete is 30. Incomplete, severe is 20. Incomplete, moderate is 10. There is no mild line. Education only.

Which muscles does the note name?

Sternomastoid and trapezius. The note says the rating is dependent upon loss of motor function of those muscles.

Does a bilateral rating use the bilateral factor?

The cranial-nerve sentence says that when involvement is bilateral, combine but without the bilateral factor. The eleventh nerve is not in the special-sense list of first, second, third, fourth, sixth, and eighth nerves.
