Diagnostic code
8013
38 CFR § 4.124a lists Diagnostic Code 8013 as “Syphilis, cerebrospinal.” The title ends with a period. The rating cell is empty. DC 8014 and DC 8015 are the next two titles, and each rating cell is empty. An indented note follows DC 8015. Appendix B to Part 4 indexes “Syphilis, cerebrospinal.”
Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.124a source note prints 29 FR 6718, May 22, 1964, as amended at 40 FR 42540, Sept. 15, 1975; 41 FR 11302, Mar. 18, 1976; 43 FR 45362, Oct. 2, 1978; 54 FR 4282, Jan. 30, 1989; 54 FR 49755, Dec. 1, 1989; 55 FR 154, Jan. 3, 1990; 56 FR 51653, Oct. 15, 1991; 57 FR 24364, June 9, 1992; 70 FR 75399, Dec. 20, 2005; 73 FR 54705, Sept. 23, 2008; 73 FR 69554, Nov. 19, 2008; 76 FR 78824, Dec. 20, 2011; 79 FR 2100, Jan. 13, 2014.
The rating criteria at each level
8013, 8014, and 8015 — the three titles above the indented note
Rating %: —
Criteria (verbatim). Syphilis, cerebrospinal.
Rating %: —
Criteria (verbatim). Syphilis, meningovascular.
Rating %: —
Criteria (verbatim). Tabes dorsalis.
Rating %: —
Criteria (verbatim). Note: Rate upon the severity of convulsions, paralysis, visual impairment or psychotic involvement, etc.
| Rating % | Criteria (verbatim) |
|---|---|
| — | Syphilis, cerebrospinal. |
| — | Syphilis, meningovascular. |
| — | Tabes dorsalis. |
| — | Note: Rate upon the severity of convulsions, paralysis, visual impairment or psychotic involvement, etc. |
DC 8013, DC 8014, and DC 8015 are printed without the hanging indent. The note is the next row, and that row is indented. The note does not name DC 8013, DC 8014, or DC 8015.
The note has no comma before “or.” It ends with “etc.”
None of the three titles prints 100, 60, 30, 10, or 0.
The opening of § 4.124a, in brackets, says: “With the exceptions noted, disability from the following diseases and their residuals may be rated from 10 percent to 100 percent in proportion to the impairment of motor, sensory, or mental function. Consider especially psychotic manifestations, complete or partial loss of use of one or more extremities, speech disturbances, impairment of vision, disturbances of gait, tremors, visceral manifestations, etc., referring to the appropriate bodily system of the schedule. With partial loss of use of one or more extremities from neurological lesions, rate by comparison with the mild, moderate, severe, or complete paralysis of peripheral nerves.” DC 8013’s own cell does not print a percentage. The indented note says to rate upon the severity of convulsions, paralysis, visual impairment or psychotic involvement, etc. It does not assign those manifestations to a diagnostic code. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just. 38 CFR § 4.14 says the evaluation of the same disability under various diagnoses is to be avoided, and that the evaluation of the same manifestation under different diagnoses is to be avoided.
DC 8014 is “Syphilis, meningovascular.” DC 8015 is “Tabes dorsalis.” Both titles end with a period, and both rating cells are empty. The indented note is printed after DC 8015. DC 8017 is amyotrophic lateral sclerosis at 100. Its note says “Consider the need for special monthly compensation.” That sentence is not the DC 8013 note.
The 2026-10-01 text of 38 CFR § 3.309 does not contain “syphilis.” It lists “Other organic diseases of the nervous system.” That family name is not assigned to DC 8013. A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

What the words mean in your records
The title ends with a period
The schedule cell is “Syphilis, cerebrospinal.” The comma is in the title. The rating cell on that row is empty. Appendix B uses the same words.
The note is indented after DC 8015
The note says to rate upon the severity of convulsions, paralysis, visual impairment or psychotic involvement, etc. It does not print a percentage, and it does not name DC 8013.
The SMC sentence is on DC 8017
DC 8017’s note says to consider the need for special monthly compensation. DC 8013’s block does not print that sentence.
Evidence that speaks to each level
The DC 8013 title
What the rater needs to see. The diagnosis the schedule titles “Syphilis, cerebrospinal.” The rating cell on that title is empty.
Where that usually lives. The diagnosis in the record, read against this title rather than DC 8014 or DC 8015.
The indented note
What the rater needs to see. Convulsions, paralysis, visual impairment, or psychotic involvement, which are the manifestations the note names, when that note is the instruction being used.
Where that usually lives. The examination of those manifestations. The note does not name the diagnostic code for each one.
VA’s public DBQ list, checked October 5, 2026, names “Central Nervous System and Neuromuscular Diseases” under Neurological. The file is Central Nervous System and Neuromuscular Diseases.
The exam form
DC 8013 prints no percentage. The indented note is the rating instruction in that block. This page does not say the form replaces that note.
Related codes and how they interact
Meningovascular syphilis is DC 8014, and tabes dorsalis is DC 8015.
Those two titles also have empty rating cells. The indented note is printed after DC 8015. DC 8017 is a different code, at 100, with the special-monthly-compensation sentence.
Secondary conditions and presumptions
Section 3.309 does not contain “syphilis” and does not assign DC 8013. “Other organic diseases of the nervous system” is a separate list entry.
A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
How does VA rate cerebrospinal syphilis under DC 8013?
Under 38 CFR § 4.124a, Diagnostic Code 8013 prints “Syphilis, cerebrospinal.” and no percentage. An indented note after DC 8015 says to rate upon the severity of convulsions, paralysis, visual impairment or psychotic involvement, etc. Education only. VA decides on the evidence.
Is meningovascular syphilis the same row?
No. DC 8014 is “Syphilis, meningovascular.” DC 8015 is “Tabes dorsalis.” Each of those titles has an empty rating cell. The indented note follows DC 8015.
FAQ
What percentage does DC 8013 print?
None. “Syphilis, cerebrospinal.” has an empty rating cell. The indented note after DC 8015 says to rate upon the severity of convulsions, paralysis, visual impairment or psychotic involvement, etc. Education only.

No. The note does not name DC 8013, DC 8014, or DC 8015. It is indented, and it is printed after the DC 8015 title.
