Muscle Group XIX — VA Diagnostic Code 5319

Musculoskeletal

Diagnostic code

5319

38 CFR § 4.73 lists Diagnostic Code 5319 in the table captioned “The Torso and Neck.” The title cell is “5319 Group XIX. Function: Support and compression of abdominal wall and lower thorax; flexion and lateral motions of spine; synergists in strong downward movements of arm (1). Muscles of the abdominal wall: (1) Rectus abdominis; (2) external oblique; (3) internal oblique; (4) transversalis; (5) quadratus lumborum”. The rating cell on that title is empty. The column header for this table is “Rating.” It is not split into dominant and nondominant. Severe is 50. Moderately Severe is 30. Moderate is 10. Slight is 0. Appendix B to Part 4 indexes “Group XIX Function: Abdominal wall and lower thorax.” The catalog row cites § 4.71a. The 2026-10-01 text places this code in § 4.73.

Reviewed 2026-10-05. Text checked against the eCFR versioner full-text XML for 2026-10-01 on 2026-10-05. Title 38 was up to date as of 2026-10-01 and last amended on 2026-08-27. The § 4.73 source note prints 62 FR 30239, June 3, 1997, as amemded 85 FR 76464, Nov. 30, 2020. The word “amemded” is the spelling in that source note. The § 4.56 source note prints 62 FR 30238, June 3, 1997. The § 4.55 source note prints 62 FR 30237, June 3, 1997.

Rating %: 10

Criteria (verbatim). Moderate

Rating %: 0

Criteria (verbatim). Slight

Note (2) of § 4.73: Ratings of slight, moderate, moderately severe, or severe for diagnostic codes 5301 through 5323 will be determined based upon the criteria contained in § 4.56.

Section 4.56(c) says the cardinal signs and symptoms of muscle disability are loss of power, weakness, lowered threshold of fatigue, fatigue-pain, impairment of coordination and uncertainty of movement.

Section 4.56(d) says that under diagnostic codes 5301 through 5323, disabilities resulting from muscle injuries shall be classified as slight, moderate, moderately severe or severe as follows. There is no comma before “or severe.”

Slight, type of injury: simple wound of muscle without debridement or infection. History and complaint: service department record of superficial wound with brief treatment and return to duty; healing with good functional results; no cardinal signs or symptoms. Objective findings: minimal scar; no evidence of fascial defect, atrophy, or impaired tonus; no impairment of function or metallic fragments retained in muscle tissue.

Moderate, type of injury: through and through or deep penetrating wound of short track from a single bullet, small shell or shrapnel fragment, without explosive effect of high velocity missile, residuals of debridement, or prolonged infection. History and complaint includes a record of consistent complaint of one or more of the cardinal signs and symptoms, particularly lowered threshold of fatigue after average use, affecting the particular functions controlled by the injured muscles. Objective findings include entrance and (if present) exit scars, small or linear, indicating short track of missile through muscle tissue, and some loss of deep fascia or muscle substance or impairment of muscle tonus and loss of power or lowered threshold of fatigue when compared to the sound side.

Moderately severe, type of injury: through and through or deep penetrating wound by small high velocity missile or large low-velocity missile, with debridement, prolonged infection, or sloughing of soft parts, and intermuscular scarring. History and complaint includes hospitalization for a prolonged period for treatment of wound, consistent complaint of cardinal signs and symptoms, and, if present, evidence of inability to keep up with work requirements. Objective findings include entrance and (if present) exit scars indicating track of missile through one or more muscle groups; indications on palpation of loss of deep fascia, muscle substance, or normal firm resistance of muscles compared with sound side; and tests of strength and endurance compared with sound side that demonstrate positive evidence of impairment.

Severe, type of injury: through and through or deep penetrating wound due to high-velocity missile, or large or multiple low velocity missiles, or with shattering bone fracture or open comminuted fracture with extensive debridement, prolonged infection, or sloughing of soft parts, intermuscular binding and scarring. The history-and-complaint paragraph says the cardinal signs and symptoms are worse than those shown for moderately severe muscle injuries. Objective findings include ragged, depressed and adherent scars indicating wide damage to muscle groups in missile track; loss of deep fascia or muscle substance, or soft flabby muscles in the wound area; muscles that swell and harden abnormally in contraction; and tests of strength, endurance, or coordinated movements compared with the uninjured side that indicate severe impairment of function.

Section 4.56(d)(4)(iii) also lists signs of severe muscle disability, if present: (A) X-ray evidence of minute multiple scattered foreign bodies indicating intermuscular trauma and explosive effect of the missile; (B) adhesion of scar to one of the long bones, scapula, pelvic bones, sacrum or vertebrae, with epithelial sealing over the bone rather than true skin covering in an area where bone is normally protected by muscle; (C) diminished muscle excitability to pulsed electrical current in electrodiagnostic tests; (D) visible or measurable atrophy; (E) adaptive contraction of an opposing group of muscles; (F) atrophy of muscle groups not in the track of the missile, particularly of the trapezius and serratus in wounds of the shoulder girdle; (G) induration or atrophy of an entire muscle following simple piercing by a projectile.

38 CFR § 4.56(a) says an open comminuted fracture with muscle or tendon damage will be rated as a severe injury of the muscle group involved unless, for locations such as in the wrist or over the tibia, evidence establishes that the muscle damage is minimal. Paragraph (b) says a through-and-through injury with muscle damage shall be evaluated as no less than a moderate injury for each group of muscles damaged. Paragraph (b) hyphenates “through-and-through.” Paragraph (d)(2) says “Through and through” without those hyphens.

38 CFR § 4.55(b) says the skeletal muscles are divided into 23 muscle groups in 5 anatomical regions, and that 5 muscle groups for the torso and neck are diagnostic codes 5319 through 5323. Paragraph (a) says a muscle injury rating will not be combined with a peripheral nerve paralysis rating of the same body part, unless the injuries affect entirely different functions. Paragraph (e) says that for compensable muscle group injuries which are in the same anatomical region but do not act on the same joint, the evaluation for the most severely injured muscle group will be increased by one level and used as the combined evaluation for the affected muscle groups. Paragraph (f) says that for muscle group injuries in different anatomical regions which do not act upon ankylosed joints, each muscle group injury shall be separately rated and the ratings combined under § 4.25. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.

Note (1) of § 4.73 names loss of use of any extremity or loss of use of both buttocks and points to § 3.350, with diagnostic code 5317, Muscle Group XVII, in parentheses. The note does not name DC 5319. DC 5320 is the next code, Group XX, postural support of body. DC 5331 is compartment syndrome. Its row says to rate each affected muscle group separately and combine in accordance with § 4.25. DC 5319 is inside the 5301-through-5323 span in Note (2). DC 5331 is not.

The 2026-10-01 text of 38 CFR § 3.309 does not name Muscle Group XIX. DC 5319 prints a 0 percent line for Slight. 38 CFR § 4.14 says the evaluation of the same disability under various diagnoses is to be avoided, and that the evaluation of the same manifestation under different diagnoses is to be avoided.

What the words mean in your records

One rating column

The Torso and Neck table headers are a blank criteria column and “Rating.” Severe is 50, Moderately Severe is 30, Moderate is 10, and Slight is 0. The table does not print a dominant column and a nondominant column for Group XIX.

The four words come from § 4.56

Note (2) assigns slight, moderate, moderately severe, and severe, for diagnostic codes 5301 through 5323, to the criteria in § 4.56. The rating table capitalizes “Moderately Severe.” Note (2) says “moderately severe.”

Appendix B shortens the function

Appendix B says “Group XIX Function: Abdominal wall and lower thorax.” The § 4.73 cell also names support and compression of the abdominal wall and lower thorax, flexion and lateral motions of the spine, synergists in strong downward movements of the arm, and the five muscles, including quadratus lumborum.

Evidence that speaks to each level

50, 30, 10, or 0

What the rater needs to see. The § 4.56 classification for severe, moderately severe, moderate, or slight. The cardinal signs and symptoms in § 4.56(c) are part of that classification.

Where that usually lives. The muscle examination and the service-treatment description of the wound, when the record has one.

Another muscle group

What the rater needs to see. Section 4.55(e) and (f) are the combining rules for muscle groups in the same region and in different regions. Note (1) of § 4.73 names DC 5317 for the § 3.350 question. It does not name DC 5319.

Where that usually lives. The decision, if it rates more than one muscle group or discusses loss of use.

VA’s public DBQ list, checked October 5, 2026, names “Muscle Injuries.” The file is Muscle Injuries.

The exam form

DC 5319 prints 50, 30, 10, and 0. Section 4.56 is the classification for those four words. This page does not say the muscle form replaces either text.

Related codes and how they interact

Group XX is the next torso code.

DC 5320 is Group XX, postural support of the body. Section 4.55(b) places diagnostic codes 5319 through 5323 in the torso and neck. DC 5331, compartment syndrome, tells the reader to rate each affected muscle group and is outside the 5301-through-5323 span.

Secondary conditions and presumptions

Section 3.309 does not name Muscle Group XIX. This page does not add DC 5319 to a presumptive row.

A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

Short answers

How does VA rate Muscle Group XIX under DC 5319?

Under 38 CFR § 4.73, Group XIX is 50 percent for severe, 30 percent for moderately severe, 10 percent for moderate, and 0 percent for slight. Note (2) points those words to the criteria in § 4.56. The muscles named in the title cell include rectus abdominis, external oblique, internal oblique, transversalis, and quadratus lumborum. Education only. VA decides on the evidence.

What does § 4.56(b) say about a through-and-through injury?

A through-and-through injury with muscle damage shall be evaluated as no less than a moderate injury for each group of muscles damaged. Paragraph (b) uses hyphens in “through-and-through.” Education only.
