Musculoskeletal
Diagnostic code
5139
38 CFR § 4.71a lists Diagnostic Code 5139 under “Three digits of one hand, amputation of:”. The row is “Index, long and little.” There is no comma before “and.” The row does not end with a period. The Major column is 50. The Minor column is 40. Those cells do not carry footnote 1. Appendix B indexes “Index, long and little.” and does end with a period.

The rating criteria at each level
5139 Index, long and little — Major and Minor columns
Rating %: 50 major, 40 minor
Criteria (verbatim). Index, long and little
| Rating % | Criteria (verbatim) |
|---|---|
| 50 major, 40 minor | Index, long and little |
The heading is “Three digits of one hand, amputation of:”. The colon is on the heading. The row does not repeat “amputation of.”
DC 5138, index, long and ring, is also 50 major and 40 minor, without footnote 1. DC 5140, index, ring and little, is the same 50 and 40. DC 5141, long, ring and little, is 40 major and 30 minor.
Footnote 1, “Entitled to special monthly compensation,” is printed on DC 5126 through DC 5130. It is not printed on DC 5139. The single-finger note later in the table says the single-finger amputation ratings are the only applicable ratings for amputations of whole or part of single fingers. DC 5139 is a three-digit row, not a single-finger row.
Paragraph (a) says: “The ratings for multiple finger amputations apply to amputations at the proximal interphalangeal joints or through proximal phalanges.” Paragraph (d) says: “Amputation or resection of metacarpal bones (more than one-half the bone lost) in multiple fingers injuries will require a rating of 10 percent added to (not combined with) the ratings, multiple finger amputations, subject to the amputation rule applied to the forearm.”
38 CFR § 4.69 says handedness for the purpose of a dominant rating will be determined by the evidence of record, or by testing on VA examination. Only one hand shall be considered dominant. The injured hand, or the most severely injured hand, of an ambidextrous individual will be considered the dominant hand for rating purposes. Section 4.69 does not use the words “Major” and “Minor.” Those are the column headings.
DC 5127 is four digits including the thumb, 70 major and 60 minor, with footnote 1. DC 5138 keeps the index and long fingers and swaps the little finger for the ring finger. It prints the same 50 and 40. DC 5140 is index, ring and little, also 50 and 40. DC 5153 is the index finger alone. Its highest line is 30 major and 20 minor, with metacarpal resection.
Paragraphs (b), (c), (e), and (f) are the same multiple-finger notes that follow DC 5151. Middle-phalanx amputation follows unfavorable ankylosis. Distal amputation, other than negligible losses, follows favorable ankylosis. Combinations are rated on the representative grade. Loss of use of the hand is the paragraph (f) sentence, not a number printed on DC 5139.
38 CFR § 4.68 says the combined rating for disabilities of an extremity shall not exceed the rating for the amputation at the elective level, were amputation to be performed. For example, the combined evaluations for disabilities below the knee shall not exceed the 40 percent evaluation, diagnostic code 5165. This 40 percent rating may be further combined with evaluation for disabilities above the knee but not to exceed the above the knee amputation elective level. Painful neuroma of a stump after amputation shall be assigned the evaluation for the elective site of reamputation. The example names diagnostic code 5165. It does not name DC 5139. Paragraph (d) is the sentence that mentions the amputation rule applied to the forearm. 38 CFR § 4.7 assigns the higher rating when the disability picture more nearly approximates the criteria required for that rating. Otherwise, the lower rating will be assigned. 38 CFR § 4.6 requires each element affecting the evidence to be studied so that decisions will be equitable and just.
The code prints no 0 percent line. 38 CFR § 4.31 says that in every instance where the schedule does not provide a zero percent evaluation for a diagnostic code, a zero percent evaluation shall be assigned when the requirements for a compensable evaluation are not met. This page does not state a payment amount. The 2026-10-01 text of 38 CFR § 3.309 does not name DC 5139. A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.

What the words mean in your records
50 and 40, without the footnote
Those are the two numbers. Footnote 1 stops at DC 5130. DC 5139 does not print it.
No thumb on this row
The four-digit rows that include the thumb are 70 and 60. This three-digit row is index, long and little.
Appendix B adds a period
The rating row does not end with a period. Appendix B indexes “Index, long and little.” with a period, and without a comma before “and.”
Evidence that speaks to each level
50 major, 40 minor
What the rater needs to see. Amputation of the index, long and little fingers of one hand, at the level paragraph (a) names.
Where that usually lives. The amputation record and which three fingers remain named in the decision.
Paragraph (d), when the metacarpal loss is the one that sentence describes
What the rater needs to see. More than one-half of the metacarpal bone lost in multiple-finger injuries. The sentence adds 10 percent. It does not combine that 10 percent.
Where that usually lives. The same operative record. Footnote 1 is not on this row.
VA’s public DBQ list, checked October 5, 2026, names “Amputations” under Musculoskeletal. The file is Amputations.
The exam form
DC 5139 prints 50 major and 40 minor. It does not print footnote 1. This page does not say the form replaces that row.
Related codes and how they interact
The ring-finger swap stays at 50 and 40.
DC 5138 is index, long and ring. DC 5140 is index, ring and little. Both print 50 major and 40 minor. DC 5141, without the index finger, prints 40 and 30.
Secondary conditions and presumptions
Section 3.309 does not name this three-finger combination. DC 5139 is a rating-schedule line.
A separate question, when a presumption does not decide it, is a medical question on the facts under 38 CFR § 3.310. Paragraph (a) covers disability proximately due to or the result of a service-connected disease or injury. Paragraph (b) covers increase in severity and requires a medical baseline.
Short answers
How does VA rate amputation of the index, long and little fingers?
Under 38 CFR § 4.71a, Diagnostic Code 5139 is 50 for the major side and 40 for the minor side. The row does not print footnote 1. Education only. VA decides on the evidence.
Where are the level-of-amputation rules?
Paragraph (a) applies the multiple-finger ratings to amputations at the proximal interphalangeal joints or through proximal phalanges. Paragraphs (b) and (c) send middle-phalanx and distal amputations to the ankylosis rules. Paragraph (d) adds 10 percent for the metacarpal loss it describes.
FAQ
What are the ratings for amputation of the index, long and little fingers?
50 in the Major column and 40 in the Minor column. Those cells do not carry footnote 1. Education only. This page does not state a payment amount.
Why is footnote 1 missing on this row?
The 2026-10-01 table prints footnote 1 on DC 5126 through DC 5130. DC 5139 is a later three-digit row. The cells are 50 and 40, with no superscript.
Does the four-digit thumb row use these numbers?
No. DC 5127, thumb, index, long and ring, is 70 major and 60 minor, and it does print footnote 1. DC 5139 has no thumb.
