Is Special Monthly Pension the Same as Special Monthly Compensation (SMC)?

The short answer

No. Special Monthly Compensation (SMC) is not the same check as pension Aid and Attendance or Housebound — the add-ons people often call Special Monthly Pension. SMC is a higher rate of compensation VA describes on its special monthly compensation rates page (updated December 3, 2025). 38 CFR 3.350 opens by stating that the rates of special monthly compensation in that section are those provided under 38 U.S.C. 1114. Pension Aid and Attendance or Housebound, on VA's Aid and Attendance and Housebound page (updated May 8, 2026), are monthly payments added to a VA pension for qualified Veterans and survivors. VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid and Attendance; form revision June 2026; form page last updated May 27, 2026) is exam and evidence support VA says to use to apply for Aid and Attendance that will be added to monthly compensation or pension — not a substitute for the underlying benefit claim and not an automatic program election. The mix-up

Form 21-2680 sits on one exam sheet and names Aid and Attendance in language that covers compensation or pension. Families then treat “Aid and Attendance / Special Monthly Pension” as the same deposit as Special Monthly Compensation. That mash-up usually mixes three different VA sentences:

SMC is treated as a needs-based pension extra because the form talks about help with daily activities. VA's compensation-versus-pension page writes that disability compensation is not based on net worth or income.
Pension Aid and Attendance is treated as an SMC letter (for example L) because both phrases mention aid and attendance. VA's pension A&A page writes those payments as amounts added to a VA pension, for qualified Veterans and survivors who already get that pension.
A signed 21-2680 is treated as the whole claim, or as an automatic election of compensation versus pension. VA's form page describes applying for Aid and Attendance that will be added to monthly compensation or pension benefits. It does not say the form grants SMC-L, grants pension A&A, or picks the program by itself.

What SMC is

From VA's current special monthly compensation rates page (updated December 3, 2025): VA special monthly compensation (SMC) is a higher rate of compensation that VA pays to Veterans as well as their spouses, surviving spouses, and parents with certain needs or disabilities. That page publishes 2026 SMC rate tables. This note does not paste those monthly figures. Use the live table.

The same rates page uses letter designations as columns. You will see different columns for designations such as L versus S. It also writes, in its own words:

Levels L through O cover specific disabilities and situations
Level R may apply if you need daily help from another person for basic needs (like dressing, eating, and bathing)
Level S may apply if you can't leave the house because of your service-connected disabilities

38 CFR 3.350 is titled Special monthly compensation ratings. Its first sentence is: the rates of special monthly compensation stated in that section are those provided under 38 U.S.C. 1114. This page does not invent extra stacking rules, half-step math, or which letter applies to a veteran's facts. Disability compensation itself is the service-connected path. VA's how pension benefits and disability compensation are different page (updated June 1, 2026) writes that disability compensation is for Veterans with service-connected disabilities who served during wartime, peacetime, or both. Eligibility is based on the type and severity of disability. It isn't based on net worth or income level. Your level of disability (your disability rating) determines the amount of your disability compensation. This note does not invent a rating or a dollar amount.

What pension Aid and Attendance / Housebound is

From VA's Aid and Attendance benefits and Housebound allowance page (last updated May 8, 2026): VA Aid and Attendance or Housebound benefits provide monthly payments added to the amount of a monthly VA pension for qualified Veterans and survivors. If you need help with daily activities, or you're housebound, that page says to find out if you qualify.

Aid and Attendance, as that page writes it

You may be eligible for this benefit if you get a VA pension and you meet at least 1 of these requirements. At least 1 of these must be true:

You need another person to help you perform daily activities, like bathing, feeding, and dressing, or
You have to stay in bed—or spend a large portion of the day in bed—because of illness, or
You're a patient in a nursing home due to the loss of mental or physical abilities related to a disability, or
Your eyesight is limited (even with glasses or contact lenses you have only 5/200 or less in both eyes; or concentric contraction of the visual field to 5 degrees or less)

Housebound, as that page writes it

You may be eligible for this benefit if you get a VA pension and you spend most of your time in your home because of a permanent disability (a disability that doesn't go away).

Note on that same page: you can't get Aid and Attendance benefits and Housebound benefits at the same time. That is VA's sentence about those pension add-ons. This note does not turn it into an SMC stacking chart.

Getting the add-on requires getting a VA pension first, as that page writes the eligibility sentences around “if you get a VA pension.” Pension is the needs-based wartime path. The compensation-versus-pension page writes that pension benefits are for Veterans who served during wartime; eligibility is based on financial need; VA considers your and your dependents' net worth and annual income, and your age or the severity of any disability you may have (it doesn't have to be service connected). Your income determines the amount of your pension payments. This note does not invent a MAPR figure. Use the live pension pages, including the A&A page, for current amounts.

What Form 21-2680 actually is

From the VA Form 21-2680 page (form revision June 2026; last updated May 27, 2026):

The pension A&A page says you can apply for VA Aid and Attendance or Housebound benefits online, by mail, or in person. For the online and mail paths, it writes that a medical examiner must fill out the examination information section. For mail, it names VA Form 21-2680 and points to the Pension Intake Center address on that live page. This note does not freeze a mailing address.

That is evidence and exam support for Aid and Attendance that VA says will be added to monthly compensation or pension. It is not the underlying compensation claim. It is not the underlying pension claim. It is not a magic checkbox that elects SMC versus pension. A signed 21-2680 does not, by itself, guarantee SMC-L or pension Aid and Attendance.

Nursing-home companion: Form 21-0779

The same pension A&A page says: if you're in a nursing home, you'll also need to fill out a Request for Nursing Home Information in Connection with Claim for Aid and Attendance (VA Form 21-0779). The VA Form 21-0779 page (form revision September 2023; last updated March 26, 2026) names that form and says to use it if you're a nursing home official to verify a Veteran or someone connected to a Veteran is a patient in a qualifying extended care facility. That is the companion those VA pages name — not a substitute for 21-2680, and not a grant.

You cannot get both pension and compensation

Quote VA's compensation-versus-pension page (updated June 1, 2026):

You can't get VA pension payments and disability compensation at the same time. If you apply for and are eligible for both, we'll pay you whichever benefit is the greater amount.

That sentence is why treating Form 21-2680 as a way to collect “the pension A&A check plus the SMC check” does not match VA.gov. This note does not calculate which amount is greater, and it does not invent a dollar comparison.

Practical checklist

Before you treat A&A as one check (educational)

Name the path in the papers in your hands: disability compensation (the SMC rates / 38 CFR 3.350 path) versus a VA pension (the Aid and Attendance / Housebound add-on path). This page does not pick that path for you.
If the facts match SMC as VA writes it, start from the SMC rates page and 38 CFR 3.350. The letter ladder overview is /blog/smc-special-monthly-compensation. Do not paste a blog's old dollar column over the live table.
